狼疮脑病合并皮质层状坏死一例

摘　　要：1例30岁青年男性,既往有10年系统性红斑狼疮病史,本次发病在3个月内连续出现3次新发脑梗死,诊断为狼疮脑病。在患者第2次发病时,颅脑磁共振局部出现皮质层状坏死。第3次发病时,颅脑磁共振呈现出广泛的皮质层状坏死。在出现皮质层状坏死后,患者症状逐渐加重,最终死亡。皮质层状坏死可能为狼疮脑病预后不良的因素之一。We described a 30-year-old man with 10 years of systemic lupus erythematous（ SLE）. He developed cerebral infarction 3 times within only 3 months, and was diagnosed with lupus encephalopathy. Cranial magnetic reso- nance imaging（MRI） showed local laminar cortical necrosis（CLN） in the second attack and a diffuse CLN in the third attack. Symptoms became more severe associated with the appearance of CLN and he died several days later. Central nervous system lupus with CLN may predict poor prognosis.
