万古霉素磷酸钙骨水泥治疗慢性骨髓炎的临床效果分析

摘　　要：目的探讨万古霉素磷酸钙骨水泥治疗慢性骨髓炎的临床效果。方法选取2014年6月-2017年6月入院治疗的70例慢性骨髓炎患者,随机分为两组,万古霉素磷酸钙组患者给予万古霉素磷酸钙骨水泥治疗,灌注冲洗组患者给予灌注冲洗治疗,比较两组患者围手术期临床指标、总有效率、复发率、功能评价与相关疾病指标。结果万古霉素磷酸钙组患者术后下地时间、抗生素应用时间、完全愈合时间与住院时间均显著低于灌注冲洗组;总有效率显著高于灌注冲洗组,复发率明显低于灌注冲洗组;功能评价明显优于灌注冲洗组;治疗后,万古霉素磷酸钙组患者的RBC、HGB、HCT、LYMPH与ALB水平均显著高于灌注冲洗组,ERS、WBC、NEUT%与CRP均显著低于灌注冲洗组,差异均有统计学意义(P<0.05)。结论万古霉素磷酸钙骨水泥治疗慢性骨髓炎的临床效果显著,具有借鉴意义。Objective To investigate the clinical effect of vancomycin calcium phosphate cement against chronic osteomyelitis.Methods The 70 patients with chronic osteomyelitis who were admitted to our hospital between June 2014 and June 2017 were randomly divided into two groups. The patients in vancomycin calcium phosphate group were treated with vancomycin calcium phosphate cement,while those in the irrigation group were given irrigation. The perioperative clinical indicators,total effective rate,recurrence rate,functional evaluation and related disease indicators were compared between the two groups.Results The postoperative time of ambulation,duration of use of antibiotics,complete healing time and length of hospital stay were significantly shorter in the vancomycin calcium phosphate group than those in the irrigation group. The total effective rate was significantly higher than that of the irrigation group,the recurrence rate was significantly lower,and functional evaluation was significantly better than that in the irrigation group. After treatment,the levels of RBC,HGB,HCT,LYMPH and ALB in the vancomycin calcium phosphate group were significantly higher than those of the irrigation group. ERS,WBC,NEUT% and CRP of the vancomycin calcium phosphate group were significantly lower than those of the irrigation group,with statistically significant difference（ P0.05）. Conclusion The clinical effect of vancomycin calcium phosphate cement in the treatment of chronic osteomyelitis is significant.
