化痰祛瘀汤联合加味化痰通络汤治疗缺血性中风病痰瘀阻络型临床研究

摘　　要：目的探讨化痰祛瘀汤联合加味化痰通络汤治疗海南地区缺血性中风病痰瘀阻络型的临床疗效。方法选取海南地区医院2016年1月至2017年9月收治的痰瘀阻络型缺血性中风病患者120例,按随机数字表法分为对照组和观察组,各60例。对照组患者给予拜阿司匹林、硝苯地平控释片等脱水降颅压、降血压、降血糖、调血脂等对症治疗,观察组患者在对照组治疗基础上给予化痰祛瘀汤联合加味化痰通络汤治疗。结果治疗后,两组患者的中医证候积分均较治疗前降低(P<0.05),且观察组明显低于对照组(P<0.05);观察组总有效率为83.33%,明显高于对照组的66.67%(P<0.05);两组患者神经功能缺损评分和Barthelr指数均较治疗前明显改善,且观察组明显优于对照组(P<0.05)。结论化痰祛瘀汤联合加味化痰通络汤治疗海南地区缺血性中风病痰瘀阻络型较单纯西药治疗疗效更显著,可明显减轻脑缺血后炎症损伤,缓解神经功能缺损,提高日常生活能力,值得推广。Objective To investigate the clinical efi^ct of Huatan Quyu Decoction combined with modified Huatan Tongluo Decoction in the treatment of ischemic stroke（phlegm and blood stasis type） in Hainan area. Methods Totally 120 patients with ischemic stroke （phlegm and blood stasis type） admitted to the hospitals in Hainan area from January 2016 to September 2017 were selected and divided into the control group and the observation group according to the random number table method, 60 cases in each group. The control group was given aspirin, Nifedipine Controlled Release Tablets and other symptomatic treatment such as dehydration, lowering blood pressure, lowering blood pressure, lowering blood sugar, regulating blood lipid, on this basis, the observation group was treated with Huatan Quyu Decoction and modified Huatan Tongluo Decoction. Results After treatment, the TCM syndrome scores of the two groups were lower than those before treatment（P 〈 0.05）, and that of the observation group was significantly lower than that of the control group （P 〈 0.05）. The total efi^ctive rate of the observation group was 83.33%, which was significantly higher than 66. 67% of the control group（ P 〈 0.05）. The NIHSS scores and Barthel indexes of the two groups were significantly improved compared with those before treatment （P 〈 0.05）, and those of the observation were significantly better than those of the control group（P 〈 0.05）. Conclusion Huatan Quyu Decoction combined with modified Huatan Tongluo Decoction in the treatment of ischemic stroke（phlegm and blood stasis type） in Hainan area has a significant effect compared with Western medicine alone. It can obviously reduce the inflammatory injury after cerebral ischemia, relieve neurological deficit and improve ability of daily life, which is worthy of promotion.
