摘 要:目的探讨血清叶酸(FA)、维生素B6(VitB6)、胱抑素C(Cys C)、同型半胱氨酸(Hcy)与糖尿病肾病病变程度的相关性。方法按糖尿病临床诊断标准,根据尿清蛋白排泄率(UAER)将202例2型糖尿病患者分为单纯糖尿病(SDM)组36例,早期糖尿病肾病(EDN)组114例,临床糖尿病肾病(CDN)组52例;另选择同期健康体检者50例作为对照组,分别测定各组研究对象血清中FA、VitB6、Cys C、Hcy水平,并进行分析。结果随着糖尿病肾病的发展,Hcy和Cys C水平显著升高,而FA和VitB6水平显著降低;血清Hcy和Cys C水平与UAER呈正相关(r=0.286、0.453,P<0.05),而FA和VitB6水平与UAER呈负相关(r=-0.278、-0.305,P<0.05)。结论血清Cys C、Hcy、FA和VitB6与糖尿病肾病密切相关,动态监测其水平可以为临床对疾病诊断和治疗提供参考依据,有助于对糖尿病肾病的早期干预和预后判断。Objective To investigate the diagnostic value of the change of serum folic acid(FA) ,vitamin B6 (VitB6) ,homocysteine(Hcy) and cystatin C (Cys C) levels with the degree of lesion in patients with type 2 di-abetes. Methods A total of 202 patients from department of endocrinology and nephrology who were hospital- ized from January 2015 to June 2016 in the General Hospital of Wanbei Coal Electric Group were selected. Ac-cording to the urinary albumin excretion rate(UAER) in clinical diagnostic criteria for diabetes,202 patients with type 2 diabetes were detected and the serum FA,VitB6 ,Hey and Cys C levels were analyzed,in which 36 patients were simple diabetes group(SDM), 114 patients were early diabetic nephropathy group(EDN), 52 pa-tients were clinical diabetic nephropathy group(CDN),and another 50 health person were control group. Re- sults With the development of diabetic nephropathy, Hcy and Cys C level increased significantly, while the FA and VitB6 were significantly reduced, positive correlation between serum Hcy and Cys C levels and UAER (r=0. 286 and 0. 453,P〈0.05). Negative correlation was found between FA and VitB6 levels and UAER (r= -0.278 and -0. 305 ,P〈0. 05). Conclusion Detection of serumCys C,Hcy,FA andVitB61evels can be used as diagnosis criteria for early diabetic nephropathy which is useful in the diagnosis of early diabetic nephropa-thy and detection the progression of diabetic nephropathy.
