摘  要:目的:探讨一氧化碳(CO)中毒患者早期中毒程度与血小板与淋巴细胞比值(PLR)、中性粒细胞与淋巴细胞比值(NLR)的关系。方法:选取本院2014年6月至2017年6月收治的160例CO早期中毒患者(观察组),依据患者中毒严重程度分为轻度组(n=43)、中度组(n=92)和重度组(n=25),另选取160例的健康体检者作为对照组,统计各组患者中性粒细胞、血小板、白细胞和淋巴细胞等计数;显微镜下观察各组中性粒细胞、血小板的形态变化,并分类200个中性粒细胞计算核突积分和毒性指数;计算各组患者PLR和NLR水平,Logistic回归模型分析PLR、NLR与CO中毒的关系。结果:不同程度早期CO中毒患者中性粒细胞、血小板、白细胞均显著高于对照组,淋巴细胞水平显著降低,差异均有统计学意义(均P<0.05);轻度组患者中性粒细胞中未见或极少见毒性颗粒,中度组和重度组患者中性粒细胞中毒颗粒逐渐增大,核突积分和毒性指数也逐渐增大,差异均有统计学意义(均P<0.05);随着中毒程度的增加,患者中PLR和NLR逐渐升高,且显著高于对照组,差异具有统计学意义(P<0.05);Logistic回归分析显示,PLR和NLR均是早期CO中毒的独立危险因素(P<0.05)。结论:早期CO患者中毒严重程度与PLR和NLR密切相关,血小板、中性粒细胞和淋巴细胞等的检测和PLR、NLR有助于早期CO中毒患者的病情评估。Objective:To explore the relationship between the degree of CO poisoning and platelet-to-lymphocyte ratio(PLR)or neutrophil-to-lymphocyte ratio(NLR).Methods:160 patients with early CO poisoning in our hospital from June 2014 to June 2017 were selected,and divided into a mild group(n=43),a moderate group(n=92)and a severe group(n=25)according to the severity of poisoning.160 healthy subjects were enrolled as controls.Neutrophils,platelets,white blood cells and lymphocytes were observed under microscope,and the PLR and NLR were calculated.Logistic regression model was established to analyze the relationship between PLR,NLR and CO poisoning.Results:The patients with CO poisoning had significantly higher levels of neutrophils,platelets and white blood cells than controls,while the lymphocytes count was lower(P〈0.05).In mild group,there were no or rare toxic granules in neutrophils,but gradually increased poisoning particles in neutrophils were found in severe and moderate group(P〈0.05).PLR and NLR in patients increased with the increase of the poisoning degree,and were higher than those in the controls(P〈0.05).Logistic analysis showed that NLR and PLR were independent risk factors for early CO poisoning(P〈0.05).Conclusion:Early CO poisoning was closely related to the PLR and NLR.Neutrophils,platelets,lymphocytes,PLR and NLR detection were valuable for the evaluation of the degree of CO poisoning.
