摘  要:目的:研究持续封闭负压引流技术(VSD)联合重组牛碱性成纤维细胞生长因子外用溶液(rb-bFGF)冲管治疗在慢性创面修复中的临床效果。方法:选取2015年4月至2017年6月在陕西省延安市人民医院行慢性创面修复的患者(包括深Ⅱ~Ⅲ度烧伤、糖尿病足溃疡、Ⅱ~Ⅲ期压疮、四肢大面积创伤)90例,按随机数字表法分为观察组(n=50,行VSD联合rb-bFGF冲管治疗)和对照组(n=40,行VSD治疗),比较两组治疗总有效率、创面愈合时间、住院时间、引流管更换次数、换药次数、局部组织液氧分压、肉芽组织中乳酸脱氢酶(LDH)、琥珀酸脱氢酶(SDH)活性、肉芽成熟度及并发症发生率。结果:观察组治疗有效率(94.00%)明显高于对照组(80.00%)(P<0.05)。与对照组比较,观察组创面愈合时间、住院时间明显缩短,引流管更换次数、换药次数明显减少,脓性分泌物发生率显著降低(P<0.05)。观察组治疗后创面局部组织液氧分压、SDH活性高于对照组,而LDH活性低于对照组(P<0.05)。观察组治疗后肉芽成熟度高于对照组(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:VSD联合rb-bFGF在促进慢性创面修复中的疗效优于单纯VSD治疗,能够纠正创面组织缺血缺氧,促进肉芽生长成熟,缩短创面愈合时间。Objective:To investigate the clinical efficacy of vacuum sealing drainage(VSD)technology combined with recombinant bovine basic fibroblast growth factor(rb-bFGF)in the treatment of chronic wounds repair.Methods:A total of 90 patients with chronic wound repair treated in our hospital from April2015 to June 2017 were selected,and randomly assigned into two groups receiving VSD(control group,n=40)or VSD combined with rb-bFGF drainage(observation group,n=50).The effective rate,wound healing time,hospital days,drainage pipe replacement times,dressing change times,interstitial fluid pressure,the activities of lactate dehydrogenase(LDH)and succinate dehydrogenase(SDH)in granulation tissue,granulation maturity and the incidence of complications were compared.Results:The response rate in the observation group was 94.00%,which was significantly higher than that in the control group(80.00%)(P〈0.05).Compared with the control group,the wound healing time,hospital days,drainage tube replacement times,dressing change times and the purulent secretion rate were decreased in the observation group(P〈0.05).After treatment,maturity of granulation,interstitial fluid pressure and the activity of SDH in the observation group were higher than those in the control group,while the activity of LDH was lower(P〈0.05).There was no significant difference in the incidence of complications between the two groups(P〉0.05).Conclusion:VSD combined with rb-bFGF was effective for chronic wound repair.This combination could correct the ischemia/hypoxia of wound tissue,promote the growth of granulation and shorten wound healing time.
