针刺联合舒马普坦治疗丛集性头痛急性发作的疗效及脑血流动力学改变

摘　　要：目的观察针刺联合舒马普坦治疗丛集性头痛急性发作的疗效及脑血流动力学改变情况。方法将入选的64例丛集性头痛急性发作患者随机分为对照组和观察组,对照组32例患者采用舒马普坦口服治疗,观察组32例患者在对照组治疗的基础上给予针刺治疗,分别观察两组患者治疗后15 min、20 min、30 min头痛的有效率,同侧球结膜充血的发生率,同时观察治疗后VAS评分、治疗前后脑血流动力学的改变。结果观察组与对照组治疗后15 min、20 min、30 min头痛的有效率分别为84.4%、90.6%、93.8%及62.5%、68.8%、75.0%,两组比较差异有统计学意义(P<0.05),观察组有效率优于对照组。两组治疗后同侧球结膜充血的发生率、VAS评分、脑血流动力学均下降,且观察组下降优于对照组,两组比较差异有统计学意义(P<0.05)。结论针刺联合舒马普坦治疗丛集性头痛急性发作疗效确切,可以显著改善患者的头痛,且能改善脑血流动力学。Objective To observe the therapeutic efficacy of acupuncture plus sumatriptan in treating acute attack of cluster headache and the change in cerebral hemodynamics. Method Sixty-four patients with acute attack of cluster headache were randomized into a control group and an observation group. Thirty-two patients in the control group were intervened by oral administration of sumatriptan while 32 patients in the observation group were additionally given acupuncture treatment. The effective rate at 15 min, 20 min and 30 min after the treatment, incidence of homolateral conjunctival congestion, Visual Analogue Scale（VAS） after the treatment and change in cerebral hemodynamics were observed. Result The effective rate for headache was respectively 84.4%, 90.6% and 93.8% at 15 min, 20 min and 30 min after the treatment in the observation group, versus 62.5%, 68.8% and 75.0% in the control group, and the between-group differences were statistically significant（P0.05）, indicating that the observation group produced a better effective rate than the control group. After the treatment, the incidence of homolateral conjunctival congestion, VAS score and cerebral hemodynamics readings all decreased in the two groups, and the decreases in the observation group were more significant than those in the control group with statistical significance（P0.05）. Conclusion Acupuncture plus sumatriptan can produce valid effectiveness in treating acute attack of cluster headache and it can significantly improve the pain and cerebral hemodynamics.
