温和灸不同状态关元穴治疗原发性痛经非随机对照研究  

Mild Moxibustion at Different-state Guanyuan(CV4) for Primary Dysmenorrhea: A Non-randomized Controlled Trial

摘　　要：目的比较温和灸热敏态与非热敏态关元穴治疗原发性痛经的临床疗效差异。方法选取72例原发性痛经患者,于疼痛发作前5 d开始对关元穴实施温和灸,每次艾灸时间45 min,并配合常规针刺,每个月经周期治疗(7±2)d,共治疗3个月经周期,疗程结束后,将患者在关元穴出现热敏化现象及在整个疗程中热敏化现象≥4次的归为热敏腧穴组(治疗组),没有出现热敏化现象或整个疗程中热敏化现象次数<4次的归为非热敏腧穴组(对照组),以COX痛经症状量表、痛经总体临床疗效进行评价。结果两组治疗后COX痛经症状量表严重程度和持续时间都较治疗前明显降低(P<0.01),且治疗组较对照组降低更明显(P<0.05);治疗组总有效率为94.6%,优于对照组的77.1%(P<0.05)。结论温和灸热敏腧穴治疗原发性痛经的临床疗效优于非热敏腧穴。Objective To compare the clinical efficacy between moxibustion at heat-sensitized and non-heatsensitized Guanyuan（CV4） point in treating primary dysmenorrhea. Method Seventy-two patients with primary dysmenorrhea started to receive mild moxibustion at Guanyuan 5 d before the onset of pain, 45 min each time in combination with ordinary acupuncture,（7±2）d treatment for each menstrual cycle and for 3 menstrual cycles in total. At the end of the treatment, those who showed heat sensitized phenomenon at Guanyuan or showed any heat sensitized phenomenon for ≥4 times were allocated to the heat sensitized acupoint group（treatment group）, and those who did not show heat sensitized phenomenon at Guanyuan or with the occurrence of any heat sensitized phenomenon 4 times were allocated to the non-heat-sensitized acupoint group（control group）. The Cox menstrual symptom scale（COX） and overall clinical efficacy for dysmenorrhea were adopted for evaluation. Result After the treatment, the COX dysmenorrhea intensity and lasting time decreased significantly in the two groups（P0.01）, and the decreases in the treatment group were more significant than those in the control group（P0.05）. The total effective rate was 94.6% in the treatment group, significantly higher than 77.1% in the control group（P0.05）. Conclusion Mild moxibustion at the heat sensitized acupoint can produce a more significant clinical efficacy than mild moxibustion at non-heatsensitized acupoint in treating primary dysmenorrhea.
