布托啡诺复合丙泊酚用于老年患者内镜下逆行胰胆管造影的效果分析

摘　　要：目的分析布托啡诺复合丙泊酚用于老年患者内镜下逆行胰胆管造影的临床价值。方法选取2016年6月至2017年6月三峡大学第二人民医院收治的84例在内镜下逆行胰胆管造影术的老年患者为研究对象,根据随机数字法分为观察组和对照组,各42例。观察组给予布托啡诺复合丙泊酚麻醉,对照组单独采用瑞芬太尼复合丙泊酚麻醉。对比两组不同时点间血流动力学变化、丙泊酚用量、患者苏醒时间及不良反应发生情况。结果对照组和观察组平均动脉压及心率均呈先降低后升高再降低的趋势,但观察组变化幅度较小,变化趋势相对平稳(P<0.05),两组各时间点血氧饱和度均无明显变化(P>0.05)。观察组丙泊酚用量少于对照组[(210.14±6.82)m L比(258.11±7.11)m L](P<0.01),两组苏醒时间比较差异无统计学意义(P>0.05)。观察组术中低血压发生率为4.76%(2/42),低于对照组的19.05%(8/42)(P<0.05)。结论布托啡诺复合丙泊酚麻醉用于老年患者内镜下逆行胰胆管造影的血流动力学稳定,且安全性高。Objective To analyze the clinical value of butorphanol anaesthesia combined with propofol anesthesia in endoscopic retrograde cholangiopancreatography in elderly patients. Methods From Jun. 2016 to Jun. 2017,84 elderly patients who underwent endoscopic retrograde cholangiopancreatography admitted to the Second People＇ s Hospital of China Three Gorges University were included in the study. The patients were divided into an observation group and a control group according to the random number method,42 cases each. The observation group was given butorphanol and propofol anesthesia. In the control group,remifentanil and propofol anesthesia were used. The changes of hemodynamics,propofol dosage,wake-up time,and adverse reactions of the two groups at different time points were compared. Results The mean arterial pressure and heart rate of the control group and the observation group first decreased,then increased,and then decreased.However,the observed changes in the observation group were smaller,and the trend was relatively stable（ P〈0. 05）. There was no significant change in blood oxygen saturation at each time point in the two groups（ P〉0. 05）. The dosage of propofol in the observation group was lower than that in the control group[（ 210. 14 ± 6. 82） m L vs（ 258. 11 ± 7. 11） m L]（ P〈0. 01）. There was no significant difference in the wake-up time between the two groups（ P〈0. 05）. The incidence of intraoperative hypotension in the observation group was 4. 76%（ 2/42）,which was lower than that in the control group[19. 05%（ 8/42） ]（ P〈0. 05）. Conclusion Butorphanol combined with propofol anesthesia for elderly patients with endoscopic retrograde cholangiopancreatography has significant clinical value,good hemodynamic stability and high safety.
