咽喉反流性疾病主要症状体征特异度敏感度相关分析

摘　　要：目的:前瞻性设计分析咽喉反流性疾病(LPRD)评分量表中主要症状体征的特异度敏感度及诊疗相关的预测。方法:选取经反流症状RSI和反流体征RFS评分量表初步诊断LPRD患者和仅有症状或仅有体征表现的患者,PPI治疗前后对比,计算统计结果。结果:就LPRD诊断来说,突出症状的敏感度81.38%、特异度53.06%,假阳性率18.62%;突出体征的敏感度86.76%、特异度64.62%,假阳性率30%;症状和体征双阳性者治疗有效预测值83.69%,仅有症状或体征者治疗无效预测值为49.06%和70%。结论:反流症状和体征的敏感度高于特异度,症状体征双量表符合标准者,PPI治疗有效率较高,单有症状或体征者有效率很低,不宜过度治疗。Objective： A prospective design analysis of the specific sensitivity and diagnosis of the main symptoms in laryngopharyngeal reflux disease（ LPRD） score scale the related predictions of diagnosis and treatment. Methods： Select the RSI and anti-fluidism RFS score scales to preliminarily diagnosed LPRD patients whom with symptomatic or only physical signs before and after the PPI treatment,apply the statistical data to calculate the result thereafter. Results： For the diagnosis of LPRD,the sensitivity of the salient symptoms was 81. 38%,the specificity was 53. 06%,and the false positive rate was 18. 62%. The sensitivity of the salient features was 86. 76%,the specificity was 64. 62%,and the false positive rate was 30%. The effective predictive value was 83. 69% for the patients who with double positive symptoms and physical signs,only those who had symptoms or physical signs patients were with an invalid predictive value of 49. 06% and 70%. Conclusion： The sensitivity of reflux symptoms and signs was higher than the specificity,and the dual-scale symptoms and signs were in line with the standards,PPI treatment is more effective,individuals symptoms or signs are with low efficiency and not suitable for overtreatment.
