摘 要:目的探讨膝骨关节炎(OA)患者血糖、血脂与血清标志物的相关性。方法收集膝OA患者169例。通过酶联免疫吸附法检测血清中反映早期膝OA的生物标志物,包括软骨糖蛋白-39(GP-39)、基质金属蛋白酶-3(MMP-3)、基质金属蛋白酶-10(MMP-10)、基质金属蛋白酶-13(MMP-13)、血清Ⅰ型胶原交联C-末端肽(CTXⅠ)和Ⅰ型胶原氨基端末肽(NTXⅠ),同时记录血生化检验中的空腹血糖(FBG)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白(HDL)、极低密度脂蛋白(VLDL)、载脂蛋白A1(Apo A1)、载脂蛋白B(Apo B)水平。将OA患者按GP-39中位数水平分为两组。结果两组人群的年龄、性别、BMI、FBG、TC、TG、HDL、VLDL、Apo A1、MMP-3、MMP-13及CTXⅠ之间差异无统计学意义。多元回归分析中,FBG、TG及VLDL与GP-39呈正相关性(OR=1.745,P=0.030;OR=1.369,P=0.050;OR=7.583,P=0.001);Apo A1与MMP-3、MMP-13呈正相关性(OR=9.747,P<0.001;OR=11.485,P<0.001);TC与MMP-10呈正相关性(OR=1.386,P=0.022);Apo B与CTXⅠ呈正相关性(OR=4.349,P=0.048);VLDL与NTXⅠ呈正相关性(OR=2.656,P=0.045)。结论膝OA患者部分血糖及血脂与血清标志物之间有相关性,提示血糖、血脂可能参与膝OA的发病机制。Objective To investigate the relationships between serum lipids and glucose levels and biomarkers in patients with knee osteoarthritis( OA). Methods One hundred and sixty-nine patients with symptomatic knee OA participated in this study. Serum biomarkers of early OA including cartilage glycoprotein-39( GP-39),matrix metalloproteinase-3( MMP-3),MMP-10,MMP-13,C-terminal telopeptide of type Ⅰ collagen( CTXⅠ) and N-terminal telopeptide of type Ⅰ collagen( NTXⅠ) were measured by enzyme-linked immunosorbent assay. Biochemical indexes including fasting blood glucose( FBG),total cholesterol( TC),total triglyceride( TG),high-density lipoprotein( HDL),very low-density lipoprotein( VLDL),apolipoprotein A1( Apo A1) and apolipoprotein B( Apo B)were recorded. The patients were divided into two groups according to the median level of GP-39. Results There were no significant differences between the two groups in terms of age,sex,body mass index,serum glucose,TC,TG,HDL,VLDL and Apo A1,and serum biomarkers including MMP-3,MMP-13 and CTXⅠ. In the multivariable regression analyses,FBG,TG and VLDL were significantly and positively associated with GP-39( OR = 1. 745,P= 0. 030; OR = 1. 369,P = 0. 050; OR = 7. 583,P = 0. 001); Apo A1 was positively associated with MMP-3 and MMP-13( OR = 9. 747,P 0. 001; OR = 11. 485,P 0. 001); TC was positively associated with MMP-10( OR =1. 386,P = 0. 022); Apo B was positively associated with CTXⅠ( OR = 4. 349,P = 0. 048) and VLDL was positively associated with NTXⅠ( OR = 2. 656,P = 2. 656). Conclusion There is a correlation between blood glucose and serum lipids and serum markers in some knee OA patients,suggesting that blood glucose and lipid may be involved in the pathogenesis of knee OA.
