四川省细菌耐药监测网2011~2016年非发酵革兰阴性杆菌的耐药性分析

摘　　要：目的调查分析2011~2016年四川地区临床分离的非发酵革兰氏阴性杆菌的分离率和对抗菌药物耐药性情况。方法采用自动化仪器法或K-B纸片扩散法对临床分离菌进行鉴定和抗菌药物敏感试验。结果从各临床标本中共分离出常见非发酵革兰氏阴性杆菌123693株。前3位依次是铜绿假单胞菌、鲍曼不动杆菌和嗜麦芽窄食单胞菌。非发酵革兰阴性菌中铜绿假单胞菌对亚胺培南的耐药率在14.2%~26.7%,而鲍曼不动杆菌对亚胺培南耐药率在51.4%~63.9%,除多粘菌素B,替加环素和米诺环素耐药率小于20.0%,大多数药物耐药率在45%以上;嗜麦芽窄食单胞菌对复方新诺明耐药率在6.3%~12.2%。结论非发酵革兰阴性杆菌的耐药率持续保持较高的水平,动态监测其耐药性变迁有助于控制耐药菌的发展和传播。Objective To investigate the separation rate and drug resistance changing of non-fermentative gram-negative bacillus of clinical isolates in Sichuan province from 2011 to 2016. Methods The Vitek-2 automatic microbial analyzer and Kirby-Bauer method were applied for bacteria identification and antimicrobial resistance.Results A total of 123,693 strains of clinical common nonfermentation gram-negative bacillus were isolated during the 6 years.The top 3 populations of the bacillus were Pseudomonas aeruginosa,Acinetobacter baumannii and Stenotrophomonas maltophilia.The percentages of Pseudomonas aeruginosa and Acinetobacter baumannii strains resistant to imipenem were ranged from 14. 2% to 26. 7% and 51. 4% to 63. 9%,respectively.The resistance rates of the two bacillus strains to most of the antibacterial drugs were more than 45% but less than 20. 0% to polymycin B,prostacyclin and minocycline.The percentages of Stenotrophomonas maltophilia strains resistant to trimethoprim-sulfamethoxazole were ranged from 6. 3 to 12. 2%. Conclusion Resistance rate of non-fermentative gram-negative bacillus keep a relative high level during the six years.Dynamic surveillance of the drug resistance changing can help to control the development and spread of drug-resistant bacteria.
