格列美脲治疗对2型糖尿病患者代谢指标及血清护骨素水平的影响

摘　　要：目的观察格列美脲对T2DM患者代谢指标及血清护骨素(OPG)水平的影响。方法选取2015年1月至2016年12月T2DM患者102例,其中将服用格列美脲治疗患者分为观察组(50例),服用格列本脲治疗患者分为对照组(52例)。分别于治疗前、治疗后12周、24周测定两组空腹血糖(FBG)、餐后2小时血糖(2 h BG)、空腹胰岛素(FINS)、糖化血红蛋白(Hb A1c)、血甘油三脂(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)、血尿酸(SUA),血清OPG水平。稳态模型评估胰岛素抵抗指数(HOMA-IR)和胰岛β细胞功能指数(HOMA-β)。结果观察组治疗后FBG、2 h BG、Hb A1c、OPG低于治疗前(P<0.05),且观察组治疗24周后患者血清OPG水平明显低于对照组(P<0.05),对照组治疗后HOMA-IR明显低于治疗前,而HOMA-β明显高于治疗前(P<0.05),观察组治疗24周后患者BMI、TC、LDL、SUA水平低于治疗前,差异均有统计学意义(P<0.05)。结论格列美脲可有效降低T2DM患者的血糖水平,改善代谢紊乱,并降低血清OPG水平,是一种安全有效的口服降糖药。Objective To access the influence of glimepiride on metabolic indexes and serum osteoprotegerin（ OPG） levels in patients with type 2 diabetes.Methods One hundred and two inpatients and outpatients with T2 DM treated in the endocrinology department of our hospital from January 2015 to December 2016 were selected. The patients were divided into observation group treated with glimepiride（ n = 50） and control group treated with glibenclamide（ n = 52）.The fasting blood glucose（ FBG）,2-hour postprandial blood glucose（ 2 h BG）,fasting insulin（ FINS）,glycated hemoglobin（ Hb A1 c）,triglyceride（ TG）,total cholesterol（ TC）,low density lipoprotein（ LDL）,serum uric acid（ SUA）,serum OPG,insulin resistance index（ HOMA-IR） and β-cell function indices（ HOMA-β） were detected before and after 12 weeks and 24 weeks of treatment.Results After treatment,FBG,2 h BG,Hb A1 c,and OPG in the observation group were decreased,and lower than those in the control group（ P〈0. 05）. HOMA-IR was significantly decreased while HOMA-β was significantly increased in the control group（ P〈0. 05）.The levels of BMI,TC,LDL,and SUA in the observation group after 24 weeks of treatment were decreased（ P〈0. 05）.Conclusion Glimepiride can effectively reduce the blood sugar level in patients with type 2 diabetes,improve the metabolic disorder,and reduce the serum OPG levels and have a certain role for the prevention of vascular complications.It is a kind of safe and effective oral medications.
