吗替麦考酚酯联合小剂量激素对系统性红斑狼疮的治疗效果和安全性分析

摘　　要：目的探讨吗替麦考酚酯联合激素对重症系统性红斑狼疮(SLE)儿童的疗效和安全性。方法选取2014年6月到2017年6月在我院治疗的重症SLE患儿22例,随机分为对照组和观察组,每组11例,均予甲泼尼龙冲击,对照组采用环孢素联合糖皮质激素治疗,观察组使用吗替麦考酚酯联合糖皮质激素治疗。比较两组患儿免疫功能及不良反应发生情况。结果经过治疗,观察组与对照组临床有效率无显著差异(P>0.05)。治疗后两组CD8+水平明显降低P<0.05,CD3^+、CD4^+水平明显升高P<0.05,但两组间比较差异无统计学意义(P>0.05)。观察组不良反应发生率显著低于对照组(P<0.05)。结论甲泼尼龙冲击治疗后,改用吗替麦考酚酯联合小剂量甲泼尼龙治疗重症SLE患儿效果显著,不良反应小。Objective： To investigate the efficacy of mycophenolate mofetil combined with small dose hormone in the treatment of severe SLE children. Methods： 22 cases of severe SLE children were selected as research objects. All the children were treated with prednisone after admission for 3 weeks. After 3 weeks,all children were divided into observation group and control group（ all n = 11）. The control group was treated with cyclosporin combined with small dose of glucocorticoid,the observation group were treated with mycophenol ester combined with small dose glucocorticoid. The clinical efficacy,immune function and adverse reaction of 2 groups were compared. Results： The clinical efficiency of observation group was significantly higher than control group（ P〈0. 05）. After treatment,the CD8^＋in 2 groups were significantly decreased（ P〉0. 05）,and the CD3^＋,CD4^＋increased significantly（ P〈0. 05）,but there was no statistical difference between the two groups（ P〉0. 05）. The incidence of adverse reactions in observation group was significantly lower than control group（ P〈0. 05）. Conclusion： After treatment with large dose of methylprednisolone,mycophenolate mofetil combined with small dose of methylprednisolone in the treatment of children with severe SLE has good effect and less adverse reaction.
