摘　　要：目的探讨乳腺癌主要照顾者的负性自动思维在其照顾负担与抑郁情绪中的作用,为乳腺癌主要照顾者不良情绪的干预提供依据。方法采用方便抽样法选取2017年1-6月在宜宾市第二人民医院住院的乳腺癌患者的主要照顾者112例作为研究对象,应用一般资料调查表、负性自动思维问卷(the automatic thoughts questionnaire,ATQ)、照顾者反应评估(caregiver reaction assessment,CRA)量表、抑郁自评量表(self-rating depression scale,SDS)对其进行调查。结果乳腺癌患者主要照顾者存在抑郁症状(≥50分)47例(41.96%)。相关分析显示,CRA问卷的IHS维度、IF维度、LFS维度得分均与ATQ得分呈正相关(P<0.05);ATQ得分与SDS得分呈正相关(P<0.05)。中介作用分析显示,ATQ得分对主要照顾者心理抑郁起到了显著的中介作用,其中介效应的效果量为45.9%。结论负性自动思维是乳腺癌主要照顾者健康与时间负荷、经济负荷、缺乏家庭支持与抑郁情绪的中介变量。Objective To investigate the effect of negative automatic thoughts on the relationship between stress and depression in primary caregivers of breast cancer patients,and to provide references for interventions of negative emotion in caregivers.Methods 112 primary caregivers of patients with breast cancer in our hospital from January 2017 to June were selected by convenience sampling,and they were investigated by general information questionnaire,the automatic thoughts questionnaire（ATQ）,caregiver reaction assessment scale（CRA）,and self-rating depression scale（SDS）.Results 47（41.96%）primary caregivers showed the symptom of depression（≥50 points）.Correlation analysis showed that the scores of IHS dimensions,IF dimensions and LFS dimensions of CRA questionnaire were positively correlated with ATQ score（P〈0.05）;there was a positive correlation between ATQ score and SDS score（P〈0.05）.The ATQ score played a significant mediating role in the prediction of psychological depression,and the mediating effect occurred 45.9%.Conclusions Negative automatic thoughts are the mediator between health and schedule burden,economic burden,lack of family support and depression in primary caregivers of patients with breast cancer.
