摘 要:目的调查军队三级综合性医院专科护士工作与管理现状。方法 2016年1-3月,便利抽样法选取30所军队医院专科护士503名为研究对象。采用自行设计的调查问卷对其工作与管理现状进行调查。结果 503名专科护士分布于26个专科领域。有90名从事重症监护工作,占17.89%。在担任工作方面,有89.86%的专科护士从事一般性临床护理工作,仅有4.17%的专科护士承担专科护理门诊工作。在各项能力中,专科护士自评专科护理技能达标人数为395名,占78.53%,在各项能力中排名第一;科研学术能力达标人数最少,为163名,占32.41%。在取得专科护士资格方面,503名专科护士中,有458名专科护士通过培训取得证书后在专业机构认证并进行专科进修;有246名专业护士通过军委后勤保障部卫生局进行认证,占48.91%;对于再次认证年限年限,专科护士的意见不一。在现行待遇方面,369名专科护士在工作中得到重用,占73.36%;179名专科护士对当前的薪酬待遇满意,占35.59%。在职业发展认同度的10个条目中,以"设立专科护士专职岗位可为护士提供更好的职业发展平台和前景"评分最高,为(4.72±0.53)分;以"我所从事的护理工作能够发挥专科护士作用"评分最低,为(4.24±0.80)分。结论大部分医院未设置专职专科护士岗位,工作职责及待遇不明确,其使用与管理有待规范。Objective To investigate the current work status of the nurse specialists in Class Ⅲ general military hospital.Methods By convenience sampling,503 nurse specialists from 30 military hospitals were selected and investigated by using self-designed questionnaire.Results The 503 nurse specialists were allocated from 26 fields.90 nurses were from ICU,which accounted for 17.89%.89.86% of nurse specialists engaged in clinical general nursing work,and only 4.17% worked in specialist care clinics.395 nurses passed the self-assessment for nursing skills,which accounted for 78.53%,and ranked the top one among other dimensions.The pass rate of research academic abilities ranked the lowest,with 163 nurses,accounted for 32.41%.458 nurses were awarded the nursing certification through training,and they had further education as well.246 nurses were awarded the certification by Health Bureau of Logistic Department of Military Commission,which accounted for 48.91%.Nurse specialists held different opinions toward re-certify the certification.369 nurses' work was recognized by the manager,which accounted for 73.36%.179 nurse specialists were satisfied with the current benefits package,which accounted for 35.59%.Among 10 dimensions of professional development recognition,‘creating nurse specialists' position can provide a better professional development platform for nurses' was ranked the highest(4.72±0.53),‘my work has significant effect on patients as a nurse specialist'was ranked the lowest(4.24±0.80).Conclusions Most of the hospitals do not have the position of nurse specialists,the responsibilities and benefits of nurse specialists are not clear,which call for a standardized management policy.
