自拟固脾止泻汤治疗抗生素致菌群失调性腹泻临床疗效分析

摘　　要：目的:采用自拟固脾止泻汤治疗老年抗生素致菌群失调性腹泻,并观察其临床疗效。方法:选取我院2016年6月—2017年10月收治确诊为伴有菌群失调抗生素相关性腹泻老年患者60例,按照随机数字表法分为对照组和观察组各30例,对照组患者给予肠内营养、抗感染、维持电解质平衡以及微生态制剂等常规内科治疗,观察组患者在对照组基础上给予自拟固脾止泻汤,比较两组患者临床疗效、药物起效时间以及治疗前后的腹泻次数、肠鸣音次数,记录两组患者治疗前后不同时间段的腹泻程度积分情况。结果:观察组患者临床有效率为93.33%,明显高于对照组的73.33%(P<0.05),治疗后观察组患者腹泻次数、肠鸣音次数明显少于对照组(P<0.05),观察组药物起效时间短于对照组的(P<0.05),两组患者治疗前腹泻程度积分比较差异无统计学意义(P<0.05),观察组治疗1 d时腹泻程度积分数值略低于对照组,但差异无统计学意义(P>0.05),观察组患者治疗3 d、7 d的腹泻程度积分明显低于对照组(P<0.05)。结论:采用自拟固脾止泻汤治疗抗生素致菌群失调性腹泻临床疗效显著。Objective： To observe the clinical efficacy of self-made Gupi Zhixie Decoction treating antibiotic-induced dysbacteriosis diarrhea in the aged. Methods： 60 elderly patients with antibiotic-induced dysbacteriosis diarrhea were randomly divided into the control group（ n = 30） and the observation group（ n = 30）. The control group was given routine internal medication,including enteral nutrition,anti-infection,maintaining electrolyte balance,and probiotics;on which basis,the observation group was also treated with self-made Gupi Zhixie Decoction. Clinical efficacy,onset time of medication,times of diarrhea and bowel sounds before and after treatment were compared between the two groups. The score of diarrhea was also recorded before and after treatment in both groups. Results： The clinical effective rate was 93. 33% in the observation group,which was significantly higher than 73. 33% in the control group（ P〈0. 05）. Times of diarrhea and bowel sounds were greatly less in the observation group than those in the control group after treatment（ P〈0. 05）. Onset time of medication came earlier in the observation group compared to that in the control group（ P〈0. 05）. There was no significant difference in diarrhea score between the two groups before treatment（ P〉0. 05）; it was slightly lower in the observation group than that in the control group after one-day treatment,but with no statistical difference（ P〉0. 05）; it was significant lower in the observation group than that in the control group after three-day treatment and after seven-day treatment（ P〈0. 05）. Conclusion： Self-made Gupi Zhixie Decoction has remarkable effect on antibiotic-induced dysbacteriosis diarrhea.
