包皮环切吻合器与商环包皮环切术治疗成人包皮口狭窄的疗效比较

摘　　要：目的比较包皮环切吻合器与商环行包皮环切术治疗成人包皮口狭窄的临床疗效。方法 143例包皮口狭窄患者分为包皮环切吻合器组和商环包皮环切组,比较两组手术时间、术后血肿、切口裂开、创口愈合时间、术后止痛次数、术后换药次数、包皮口再狭窄率及患者满意度的差异。结果两组患者在手术时间和术后血肿发生率方面无显著差异。包皮环切吻合器组患者总体满意度高于商环包皮环切组(分别为95.9%和73.9%,P<0.05),其创面愈合时间[分别为(7.6±3.5)天和(11.5±4.3)天]、术后使用止痛药物次数[分别为(1.6±1.1)次和(5.2±2.5)次]、术后换药次数[分别为(2.23±0.36)次和(7.73±2.69)次]和包皮口再狭窄发生率(分别为0和4.3%)均显著优于商环治疗组(P<0.05)。商环治疗组切口裂开率11.5%(8/69),包皮环切吻合器组切口裂开率为0%(0/74),差异具有显著性(P<0.05)。结论包皮环切吻合器用于治疗包皮口狭窄具有手术时间短、美观、痛苦少、恢复快的优点,值得临床推广。Objective To compare the clinical effects between stapler circumcision and shang ring circumcision for adult prepuce stenosis patients. Methods 143 patients were divided into stapler circumcision group and shang ring circumcision group. The operation time,postoperative hematoma,incision dehiscence,postoperative pain-relief drug taken,overall satisfactory score were compared between these two groups. Results There was no significant difference between operation time and postoperative hematoma in these two groups. The overall satisfactory score of stapler circumcision group was higher than that of shang ring circumcision group（ 95. 9% vs73. 9%,P〈0. 05）. The postoperative pain-relief drug taken [（ 1. 6 ± 1. 1） times vs（ 5. 2 ± 2. 5） times]was less comparing to that of shang ring circumcision group（ P〈0. 05）. The incidence of incision dehiscence in shang ring circumcision group was significantly higher than that of stapler circumcision group（ 11. 5% vs 0%,P〈0. 05）. Conclusions Stapler circumcision shortened the operation time,reduced the postoperative pain and improved the patients＇ satisfaction,and it is a recommendable alternative for the treatment of adult prepuce stenosis.
