血清β-HCG、孕酮及雌二醇对宫外孕的诊断意义

摘　　要：目的研究血清β-HCG、孕酮、雌二醇对宫外孕的诊断意义。方法方便选择2016年2月—2017年2月重庆市长寿区第三人民医院收治的病理确诊为宫外孕的孕妇62例作为观察组,同期选择正常妊娠的孕妇62名作为对照组。采用电化学发光法对所有孕妇进行检测,对比两组孕妇血清β-HCG、孕酮、雌二醇指标水平,然后分别分析以上三项指标单独、联合诊断宫外孕的准确率。结果观察组孕妇血清β-HCG水平为(549.59±168.46)UI/L、孕酮水平为(317.64±164.39)nmol/L、雌二醇水平为(4 516.09±1 649.67)pmol/m L,均低于对照组孕妇(t=4.55、5.25、6.64,P<0.05)。β-HCG、孕酮、雌二醇单独诊断宫外孕准确率分别为74.19%、41.94%、37.10%;三项指标联合诊断宫外孕准确率为96.77%,显著高于单独指标诊断(P<0.05)。结论血清β-HCG、孕酮、雌二醇等指标,均可作为宫外孕诊断的评价指标,将三项指标联合应用,能够取得更为准确的诊断结果。Objective This paper tries to study the significance of serum β-HCG, progesterone, and estradiol in the diagnosis of ectopic pregnancy. Methods From February 2016 to February 2017, 62 cases of pregnant women admitted to the Third People＇s Hospital of Changshou District as ectopic pregnancy were convenient selected as the observation group. In the same period, 62 cases of normal pregnant women were selected as the control group. Electrochemiluminescence was used to detect all pregnant women. Serum levels of β-HCG, progesterone, and estradiol were compared between the two groups of pregnant women. The accuracy of these three indicators alone and in conjunction with the diagnosis of ectopic pregnancy was analyzed. Results The serum β-HCG level in the observation group was（549.59±168.46）UI/L, the progesterone level was（317.64±164.39）nmol/L, and the estradiol level was（4 516.09±1 649.67）pmol/L, which was lower than that of the control group.（t=4.55, 5.25, 6.64, P〈0.05）. The accuracy rates of β-HCG, progesterone, and estradiol alone in the diagnosis of ectopic pregnancy were 74.19%, 41.94%, and 37.10%, respectively. The accuracy of the combined use of the three indicators in the diagnosis of ectopic pregnancy was 96.77%, which was significantly higher than that of the individual indicators（P〈0.05）. Conclusion Serum β-HCG, progesterone, estradiol and other indicators can be used as evaluation indicators for the diagnosis of ectopic pregnancy. The combination of three indicators can achieve more accurate diagnosis.
