单向倒刺线在腹腔镜胆总管探查术中的应用研究

摘　　要：目的研究单向倒刺线在腹腔镜胆总管探查手术中的应用效果。方法选取江苏大学附属武进医院2016年1月至2017年10月期间行腹腔镜胆总管探查术并用单向倒刺线连续缝合胆总管的病例50例作为倒刺线组,选取同期50例行常规可吸收线间断缝合胆总管的病例50例作为常规组,比较两组胆总管缝合时间、术中出血量、术后肠功能恢复时间、胆漏发生率、伤口感染率、住院费用、住院时间等。结果倒刺线组缝合时间(13.7±1.9)min,短于常规组(24.9±2.8)min(t=-16.113,P<0.001);倒刺线组胆漏发生率为0,而常规组12%,差异有统计学意义(P=0.027);两组术中出血量、术后肠功能恢复时间、术后切口感染、住院时间、住院费用等无统计学差异(P>0.05)。结论单向倒刺线连续缝合应用于腹腔镜胆总管探查术简单、方便、安全,并减少胆漏发生率,可作为腹腔镜下胆总管缝合的常规术式。Objective To explore the application of continuous suturing by single needle barbed suture in laparoscopic common bile duct exploration.Methods From Jan.2016 to Oct.2017,laparoscopic common bile duct exploration were performed either by single needle barbed suture with continuous suturing（50 cases,single needle barbed suture group） or by absorbable suture with conventional interrupted suturing（50 cases,control group）.The common bile duct suture time,intraoperative blood loss,postoperative recovery of intestinal function,bile leakage incidence,wound infection incidence,cost of hospitalization,as well as length of hospitalization were compared between the two groups.Results For single needle barbed suture group,shorter suturing time [（13.7±1.9） min vs（24.9±2.8） min,t=-16.113,P〈0.001],and less bile leakage incidence（0 vs12%,P=0.027） were found compared with control group.There was no significant difference in intraoperative blood loss,postoperative recovery of intestinal function,the incidence of wound infection,cost of hospitalization,length of hospitalization between the two groups（P〉0.05）.Conclusion Continuous suturing with single needle barbed suture in laparoscopic common bile duct exploration is simple,convenient and safe,with the advantage of less biliary leakage incidence,which can be used as a routine operation.
