高频电刀联合咪喹莫特治疗HIV感染/AIDS合并肛周尖锐湿疣的疗效研究

摘　　要：目的探讨高频电刀联合咪喹莫特治疗HIV感染/AIDS合并肛周尖锐湿疣的疗效。方法回顾性选择无锡市疾病预防控制中心皮肤科2013年12月至2016年6月明确诊断为HIV感染/AIDS合并肛周尖锐湿疣的患者共28例,平均年龄32.43±10.13岁,按照治疗方法分为研究组和对照组,每组各14例,其中对照组再分两个亚组,对照组A和B,各7例。研究组采用高频电刀联合咪喹莫特治疗,对照组A采用高频电刀治疗,对照组B采用液氮冷冻治疗。对两组患者治疗后症状好转时间、生活质量评分等进行密切观察,比较各组治疗的疗效和复发情况。结果两组治疗前生活质量量表(GQOL-74)总分差异无统计学意义,治疗后,研究组GQOL-74总分显著高于对照组。两组在症状缓解时间上研究组显著低于对照组,差异具有统计学意义。随访6个月发现,研究组痊愈9例,对照组A痊愈2例,对照组B痊愈3例,研究组痊愈例数显著高于对照组(χ~2=8.341,P<0.001)。研究组有效率为92.86%,高于对照组,但差异无统计学意义(χ~2=1.741,P=0.331)。研究组有2例患者出现复发(14.29%),对照组有4例患者出现复发(28.57%),研究组复发率显著低于对照组,差异具有统计学意义(χ~2=5.021,P=0.035)。两组患者的不良反应发生情况差异无统计学意义(χ~2=0.587,P=0.783)。结论运用高频电刀联合咪喹莫特治疗HIV感染/AIDS合并肛周尖锐湿疣的疗效优于单用微波或高频电刀治疗,值得临床推广应用。Objective To investigate the efficacy of high frequency electric knife combined with imiquimod in the treatment of HIV infection/AIDS combined with perianal condyloma acuminatum. Methods A total of 28 patients from department of dermatology in Center for DiseaseControl and Prevention of Wuxi and diagnosed with HIV/AIDS combined with perianal condyloma were selected retrospectively from December2013 to June 2016,with mean age 32. 43 ± 10. 13 years. According to random number method,they were randomly divided into study group（ n =14） and control group（ n = 14）. The control group was divided into control group A（ n = 7） and control group B（ n = 7）. The study group were treated with high frequency electric knife combined with imiquimod,the control group A was treated with high frequency electric knife,and the control group B was treated with liquid nitrogen frozen treatment. The symptoms of remission,the quality of life score were observed after treatment of HIV infection/AIDS combined with perianal condyloma. The curative effect and incidence of adverse reactions were also compared. Results The total score of GQOL-74 had no difference between the groups before treatment. After treatment,the total score of GQOL-74 in the study group was significantly higher than that in the control group. The difference in symptom relief in the two groups were significant statistically as well,with the study group significantly lower than the control group. After 6 months follow-up,9 cases recovered in the study group,but in the control group A 2 cases recovered,and 3 cases in the control group B. The number of cured case in the study group was significantly higher than the control group（ χ~2= 8. 341,P〈0. 001）. The effective rate in the study group was 92. 86%,which was higher than that of the control group,but there was no significant difference（ χ~2= 1. 741,P = 0. 331）.
