不同内固定方式治疗胫骨平台骨折的疗效对比

摘　　要：目的探讨不同内固定方式治疗胫骨平台骨折的临床疗效。方法选取2014年1月至2017年1月收治的123例胫骨平台骨折患者,根据内固定方式不同分为锁定钢板固定组(58例)和解剖钢板内固定组(65例),随访1年,对比分析患者手术情况、术后效果以及并发症情况。结果锁定钢板固定组患者住院时间、手术时间、术中出血量明显低于解剖钢板内固定组,差异具有统计学意义(P<0.05);锁定钢板固定组骨折愈合时间和完全负重时间与解剖钢板内固定组比较差异无统计学意义(P>0.05);锁定钢板固定组内翻角为(84.0±2.0)°,解剖钢板内固定组为(82.5±1.8)°,差异无统计学意义(P>0.05);锁定钢板固定组后倾角为(8.3±1.9)°,解剖钢板内固定组后倾角为(8.5±2.2)°,差异无统计学意义(P>0.05);锁定钢板固定组治疗的优良率明显高于解剖钢板内固定组,2组比较差异有统计学意义(P<0.05)。结论对于胫骨平台骨折的治疗,锁定钢板内固定的手术方法能获得更好的疗效,对于患者术后恢复有积极意义。Objective To explore the clinical efficacy of different internal fixation methods in treatment of tibial plateau fractures.Methods During the period from January 2014 and January 2017,123 patients with tibial plateau fracture were chosen and divided into a locking plate fixation group（ n = 58 cases） and an anatomic plate internal fixation group（ n = 65 cases） according to internal fixation Methods;after operation,the patients were followed up for 1 year. The operation,postoperative effect and complications were compared among the patients. Results The operation time,intraoperative blood loss and average length of hospital stay in locking plate fixation group were less than that of the anatomic plate internal fixation group,and the difference was statistically significant（ P 〈0. 05）. The difference between the two groups in terms of healing time and full load time was of no statistical significance（ P 〉0. 05）. In locking plate fixation group,the varus angle was（ 84. 0 ± 2. 0） ° while it was（ 82. 5 ± 1. 8） ° in the anatomic plate internal fixation group,and the difference was also of no statistical significance（ P 〉0. 05）. In locking plate fixation group,the caster angle was（ 8. 3 ± 1. 9） ° and it was（ 8. 5 ± 2. 2） ° in anatomic plate internal fixation group,the difference was also of no statistical significance（ P 〉0. 05）. The excellence rate of locking plate fixation group was obviously higher than that of anatomic plate internal fixation groupand the difference between them was statistically significant（ P〈 0. 05）. Conclusion For the treatment of tibial plateau fractures,the locking plate internal fixation operation method can get better curative effect,and it has a positive significance to patients during postoperative recovery.
