摘 要:目的探讨早期降钙素原(PCT)、血常规和C反应蛋白(CRP)联合检测在新生儿败血症诊断中的作用。方法选取2015年6月至2017年6月期间我院收治的80例败血症新生儿记为观察组,另选取我院出生的80例健康新生儿记为对照组。对比不同组别新生儿以及观察组患儿治疗前及治疗24 h后PCT和血常规、CRP的水平,并分析3种诊断方式单一检测及联合检测对新生儿败血症的诊断效能。结果观察组PCT、CRP、WBC水平均高于对照组(P<0.05),PLT水平低于对照组(P<0.05);观察组治疗24 h后,PCT、CRP、WBC水平均低于治疗前(P<0.05),PLT水平高于治疗前(P<0.05)。PCT的敏感性及特异性均高于CRP、WBC及PLT,且PCT的诊断指数和Youden指数均最高(P<0.05);PCT、CRP、WBC及PLT联合检测的敏感性、诊断指数和Youden指数均高于单一指标检测(P<0.05)。结论败血症新生儿存在PCT和血常规、CRP水平异常现象,早期PCT、血常规和CRP联合检测能够作为败血症诊断的有效方式。Objective To explore the role of early detection of procalcitonin( PCT),routine blood test and C-reactive protein( CRP) in the diagnosis of neonatal sepsis. Methods Eighty neonatal sepsis infants admitted to our hospital from June 2015 to June 2017 were selected as the observation group,and 80 healthy neonates born in our hospital were recorded as the control group. The levels of PCT,blood routine,and CRP before and after treatment in neonates and observation groups of different groups were compared,and the diagnostic efficacy of single and combined detection of three diagnostic methods for neonatal sepsis was analyzed.Results The levels of PCT,CRP and WBC in the observation group were higher than those in the control group( P〈 0. 05),and PLT levels were lower than those in the control group( P〈 0. 05). After 24 hours of treatment in the observation group,PCT,The levels of CRP and WBC were lower than before treatment( P〈 0. 05),and the levels of PLT were higher than before treatment( P〈 0. 05). The sensitivity and specificity of PCT were higher than that of CRP,WBC,and PLT,and the diagnostic index and Youden index of PCT were the highest( P〈 0. 05); the sensitivity,diagnostic index,and Youden index of combined detection of PCT,CRP,WBC,and PLT. Both were higher than single indicators( P〈 0. 05). Conclusion There are abnormalities of PCT,blood routine,and CRP levels in neonates with sepsis. The combination of early PCT,routine blood tests and CRP can be used as an effective method for the diagnosis of sepsis.
