摘  要:目的探讨原发性肾病综合征及糖皮质激素对儿童骨代谢的影响。方法选取在我院就诊的60例原发性肾病综合征患儿作为观察组,行糖皮质激素治疗;同时选取在我院进行健康体检的60例健康儿童为对照组,两组均行骨代谢水平检测,对比其检测结果。结果观察组60例经治疗后,完全缓解26例,部分缓解24例,无效10例,患者治疗总有效率为83.3%。与对照组比较,观察组患儿入院时及治疗4周后血清钙水平、血清骨钙素(BGP)、血清骨碱性磷酸酶(BALP)、甲状旁腺素(PTH)等指标均存在明显差异(P<0.05);且观察组患儿入院时与治疗4周后对比,BGP、BALP、PTH等指标均存在显著差异(P<0.05)。结论原发性肾病综合征患儿实施糖皮质激素治疗对其骨代谢造成影响,临床上需予以重视。Objective To investigate the effect of primary nephrotic syndrome and glucocorticoid on bone metabolism in children. Methods Sixty children with primary nephrotic syndrome who were treated in our hospital were selected for study. The observation group was treated with glucocorticoids. At the same time,60 healthy children in our hospital were selected as the control group. All bone metabolic levels were tested and the results were compared. Results After the treatment in the observation group,there were 26 cases completely relieved,24 cases partially relieved,and 10 cases failed. The total effective rate of treatment was 83. 3 %. Compared with the control group,serum calcium levels,serum BGP,bone alkaline phosphatase( BALP),parathyroid hormone( PTH) and other indicators were all significant in the observation group when they were admitted to hospital and after 4 weeks of treatment. Differences( P〈 0. 05); BGP,BALP,PTH and other indicators were significantly different( P〈 0. 05) in the observation group compared with the 4 weeks after treatment. Conclusions The effect of glucocorticoid treatment on bone metabolism in children with primary nephrotic syndrome must be considered clinically.
