氩等离子体凝固治疗在溃疡坏死型气管支气管结核早期治疗中的临床价值

摘　　要：目的探讨氩等离子体凝固治疗在溃疡坏死型气管支气管结核早期治疗中的临床价值。方法回顾性分析2013年01月-2015年10月于我院诊断为溃疡坏死型气管支气管结核100例患者,其中对照组为经支气管镜下冷冻+活检钳清理及局部注药患者50例,实验组为在对照组治疗基础上早期加用氩等离子体凝固治疗患者50例,比较两组经气管镜介入治疗一月后疗效。同时记录至支气管结核病变稳定时,镜下介入治疗总次数。结果与对照组比较,实验组在临床症状、镜下病灶、影像学改变均显著改善,差异有统计学意义(P<0.05)。至气管支气管结核病灶稳定时,实验组总治疗次数显著低于对照组,差异有统计学意义(P<0.05)。结论氩等离子体凝固治疗早期干预,可提高溃疡坏死型气管支气管结核患者疗效,减少气管镜介入治疗次数,且安全性高,值得临床推广。Objective To investigate the clinical value of argon plasma coagulation therapy in early treatment of necrotizing ulcerative type in bronchial tuberculosis. Methods A retrospective analysis of 100 patients admitted to our hospital from January 2013 to October 2015 was performed. They diagnosed as ulcerative necrosis of bronchial tuberculosis clearly. The control group consisted of 50 patients who underwent endoscopic cryotherapy and biopsy and local injection. The experimental group consisted of 50 patients who were treated with argon plasma coagulation at early stage on the basis of the control group. The curative effect of the two group one month after interventional treatment with bronchoscope was analyzed. At the same time,the total number of interventional treatment was recorded at the time of stabilization of endobronchial tuberculosis. Results Compared with the control group,their clinical symptoms,endoscopic and imaging findings were significantly improved in the experimental group（ P〈0. 05）. In addition,when tracheobronchial tuberculosis was stable,the total number of treatment of the experimental group was significantly lower than that of the control group（ P〈0. 05）. Conclusion Argon plasma coagulation therapy can improve the curative effect of patients with ulcerative necrosis of bronchial tuberculosis at early stage,it can reduce the total number of interventional treatment of bronchoscopy. At the same time,it is safe and worthy of clinical promotion.
