栝楼桂枝汤经穴导入对脑卒中后痉挛性足下垂的疗效观察

摘　　要：目的观察栝楼桂枝汤经穴导入对脑卒中后痉挛性足下垂的影响。方法将31例脑卒中后痉挛性足下垂患者随机分为治疗组和对照组,治疗组采取栝楼桂枝汤经穴导入治疗;对照组采取栝楼桂枝汤常规口服。观察两组治疗前后临床痉挛指数(CSI)、Berg平衡量表(BBS)、改良Barthel指数(MBI)评分变化,并利用表面肌电图采集足背屈时胫骨前肌、腓肠肌的表面肌电图积分(i EMG),计算踝背伸时协同收缩率(CR)。结果两组治疗后的CSI评分、BBS评分、MBI评分、足背屈时胫骨前肌和腓肠肌i EMG、踝背伸时CR与同组治疗前比较,差异有统计学意义(P<0.05);治疗组治疗后CSI评分、BBS评分、MBI评分,足背屈时胫骨前肌和腓肠肌i EMG、踝背伸时CR均优于对照组(P<0.05)。结论栝楼桂枝汤经穴导入能够改善脑卒中后痉挛性足下垂。Objective To observe the effect of acupoint iontophoresis with Gualou Guizhi decoction on spastic foot drop after cerebral stroke. Method Thirty-one patients with spastic foot drop after cerebral stroke were randomized into a treatment group and a control group. The treatment group was intervened by acupoint iontophoresis with Gualou Guizhi decoction; the control group was intervened by oral administration of Gualou Guizhi decoction. The two groups were evaluated using the Clinical Spasticity Index（CSI）, Berg Balance Scale（BBS） and Modified Barthel Index（MBI） before and after the intervention. The surface electromyography（s EMG） was used to collect the integrated electromyography（i EMG） of tibialis anterior muscle and gastrocnemius muscle under dorsiflexion of foot and calculate the co-contraction ratio（CR） under ankle dorsal extension. Result The CSI, BBS and MBI scores, i EMG of tibialis anterior and gastrocnemius muscles under dorsiflexion of foot, and CR under ankle dorsal extension significantly changed after the treatment in both groups（P〈0.05）. After the treatment, the CSI, BBS and MBI scores, i EMG of tibialis anterior and gastrocnemius muscles under dorsiflexion of foot, and CR under ankle dorsal extension in the treatment group were significantly better than those in the control group（P〈0.05）. Conclusion Iontophoresis with Gualou Guizhi decoction can improve spastic foot drop after cerebral stroke.
