金水宝胶囊联合厄贝沙坦片治疗早期糖尿病肾病疗效及对尿微量白蛋白、氧化应激水平的影响

摘　　要：目的观察金水宝胶囊联合厄贝沙坦片治疗早期糖尿病肾病(DN)疗效及对尿微量白蛋白(MAU)、氧化应激水平的影响。方法将60例DN患者随机分为观察组和对照组,每组30例。对照组给予厄贝沙坦片口服,观察组给予厄贝沙坦片联合金水宝胶囊口服,2组均治疗3个月后统计临床疗效,观察2组治疗前后糖代谢指标[空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(Hb A1c)]、尿微量白蛋白(MAU)、尿微量白蛋白排泄率(UAER)、肾功能指标[血清胱抑素C(Cys C)、尿素氮(BUN)、血肌酐(SCr)]及氧化应激指标[丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSHPx)]的变化情况。结果观察组总有效率明显高于对照组(P<0.05)。治疗后2组FPG、2h PG、Hb A1c、MAU、UAER、Cys C、BUN、SCr及MDA水平均明显低于治疗前(P均<0.05),且观察组治疗后上述指标水平均明显低于对照组(P均<0.05);2组治疗后SOD和GSH-Px水平均明显高于治疗前(P均<0.05),且观察组明显高于对照组(P均<0.05)。结论金水宝胶囊联合厄贝沙坦片能够显著降低早期DN患者MAU和氧化应激水平,有效保护肾功能,延缓疾病进展。Objective It is to observe the effect of Jinshui Bao Capsule combined with Irbesartan Tablets in the treatment of early diabetic nephropathy（ DN） and its influence on urinary microalbumin（ MAU） and oxidative stress. Methods Sixty patients with DN were randomly divided into the observation group and the control group,30 cases in each group. The control group was given Irbesartan Tablets orally,and the observation group was given oral administration of Irbesartan Tablets combined with Jinshui Bao Capsule. The 2 groups were treated for 3 months,and the clinical curative effect was analyzed. The index of glucose metabolism [fasting plasma glucose（ FPG）,2 hour postprandial blood glucose（ 2 h PG）,glycated hemoglobin（ HbA1c） ],microalbuminuria（ MAU）,urine microalbumin excretion rate（ UAER）,renal function index [serum cystatin C（ Cys C）,blood urea nitrogen（ BUN）,serum creatinine（ SCr） ]and oxidative stress index [malondialdehyde（ MDA）,superoxide dismutase（ SOD） and glutathione peroxidase（ GSH-Px） ]were observed before and after treatment in the 2 groups. Results The total effective rate of the observation group was significantly higher than that of the control group（ P〈0. 05）. After treatment,the levels of FPG,2 h PG,HbA1c,MAU,UAER,Cys C,BUN,SCr and MDA in the 2 groups were significantly lower than those before treatment（ all P〈0. 05）,and the above indexes in the observation group were significantly lower than those in the control group（ all P〈0. 05）; the levels of SOD and GSH-Px in the2 groups were significantly higher than those before treatment（ all P〈0. 05）,and the levels of the observation group were significantly higher than those of the control group（ all P〈0. 05）. Conclusion Jinshui Bao Capsule combined with Irbesartan Tablets can significantly reduce the levels of MAU and oxidative stress in early DN patients,and effectively protect renal function and delay disease progression.
