高龄产妇泌乳启动延迟现状及相关因素分析

摘　　要：目的了解高龄产妇泌乳启动延迟现状并分析其影响因素,为预防高龄产妇泌乳启动延迟,建立和维持母乳喂养提供参考。方法采用便利抽样法抽取在妇产科建档并分娩的高龄产妇286例,应用一般资料调查表、焦虑自评量表和抑郁自评量表进行调查。结果高龄产妇发生泌乳启动延迟112例(39.16%);焦虑发生率为75.87%,抑郁发生率为75.52%。Logistic回归分析显示,年龄、产次、焦虑和抑郁是泌乳启动延迟发生的危险因素(均P<0.01)。结论高龄产妇泌乳启动延迟的发生率较高,应重视对高龄产妇泌乳启动延迟相关因素的评估,为母乳喂养提供合理化的护理方案,有效预防和减少泌乳启动延迟的发生,以有效提高母乳喂养率。Objective To understand the status of delayed onset of lactogenesis among women of advanced reproductive age,and to analyze its related factors,so as to prevent delayed onset of lactogenesis for them,and provide reference for establishment and maintenance of breastfeeding.Methods A convenience sampling method was used to select 286 women of advanced reproductive age who had set pregnant archives and delivered in the gynecological and obstetrics department,then they were investigated using the general data inventory,Self-rating Anxiety Scale and Self-rating Depression Scale.Results Totally 112（39.16%）women suffered delayed onset of lactogenesis,the incidence of anxiety and depression was 75.87%and 75.52%respectively.Logistic regression analysis showed that,age,parity,anxiety and depression were risk factors of delayed onset of lactogenesis（P0.01 for all）.Conclusion The incidence of delayed onset of lactogenesis among women of advanced reproductive age is high.Medical staff should pay attention to the assessment of related factors of delayed onset of lactogenesis for women of advanced reproductive age,and provide reasonable nursing scheme for breastfeeding,so as to prevent and reduce the occurrence of delayed onset of lactogenesis effectively,and then effectively improve breastfeeding rate.
