摘 要:目的探讨维持性血液透析(MHD)患者血清hs-CRP、c Tn T水平以及与心血管事件发生的相关性。方法选择我院2014年3月—2017年3月间行MHD治疗的158例尿毒症患者为研究组。另选择同期健康体检者120例为对照组,比较两组患者血清hs-CRP、c Tn T水平表达情况,随访3年,记录血清hs-CRP、c Tn T水平正常及升高者心血管事件发生情况及死亡情况,并分析患者血清hs-CRP、c Tn T水平与心血管事件发生的相关性。结果研究组患者透析后血清hs-CRP、c Tn T水平均显著高于对照组(P<0.05)。随访3年中,研究组hs-CRP升高者心血管事件、死亡率及总病死率明显高于对照组(P<0.05);c Tn T升高者心血管事件、死亡率及总病死率明显高于对照组(P<0.05)。COX回归分析结果显示,血清hs-CRP及c Tn T为影响维持性血液透析患者心血管事件及预后的独立危险因素。结论 MHD患者血清hs-CRP、c Tn T是影响心血管事件及预后的独立危险因素,与心血管事件密切相关,通过对MHD患者血清hs-CRP及c Tn T进行检测,可为临床诊断及预防心血管事件提供一定的理论依据,从而改善预后。Objective: To explore the relationship between the levels of serum hs-CRP and c Tn T and the occurrence of cardiovascular events in patients under maintenance hemodialysis( MHD). Methods: 158 uremic patients who had been receiving MHD treatment in the Hospital between March 2014 and March 2017 were selected as the study group. Another 120 healthy physical examinees during the same period were selected as the control group. The levels of serum hs-CRP and c Tn T levels were compared between the two groups. The occurrence and death of cardiovascular events in patients with normal hsCRP and c Tn T levels and those with elevated levels were recorded during the 3 years of follow-up to analyze the relationship between the levels of serum hs-CRP and c Tn T and the occurrence of cardiovascular events. Results: The serum levels of hsCRP and c Tn T in the study group after dialysis were significantly higher than those in the control group( P〈0. 05). During the 3 years of follow-up,the incidence of cardiovascular events,the death rate,and the total mortality of patients with elevated hs-CRP levels in the study group were significantly higher than those in the control group( P〈0. 05),and the incidence of cardiovascular events,the death rate,and the total mortality of patients with elevated c Tn T in the study group were significantly higher than those in the control group( P〈0. 05). COX regression analysis showed that serum hs-CRP and c Tn T were independent risk factors for cardiovascular events and prognosis of patients under maintenance hemodialysis. Conclusion: Serum hs-CRP and c Tn T levels are independent risk factors for cardiovascular events and prognosis of patients under MHD,and are closely related to the occurrence of cardiovascular events. The detection of serum hsCRP and c Tn T in MHD patients can be used for clinical diagnosis and prevention of cardiovascular events,providing a certain theoretical basis for the improvement of their prognosis.
