肱骨骨折不愈合临床治疗分析

摘　　要：目的探讨分析肱骨骨折不愈合临床治疗效果。方法选取本院2014年3月~2015年10月收治的肱骨骨折不愈合患者71例作为研究对象,所有患者均予以锁定加压钢板内固定与自体髂骨植骨治疗,对治疗效果进行评价。结果71例患者切口均实现骨性愈合,骨折端对线、对位良好;平均骨折愈合时间(11.5±2.4)月,且愈合过程中未出现髓内针松动、弯曲、断裂等情况;肘关节功能恢复情况评估结果显示:优30例(42.3%),良36例(50.7%),可4例(5.6%),差1例(1.4%),优良率(93.0%);患者治疗后肘关节屈曲度、肘关节屈伸度以及MEPS评分明显优于治疗前(P<0.05);治疗满意度调查结果显示,其中满意41例(57.7%),较满意24例(33.8%),不满意6例(8.5%),治疗满意度91.5%。结论锁定加压钢板内固定与自体髂骨植骨治疗肱骨骨折不愈合的临床效果突出,值得临床应用。Objective To investigate the clinical treatment of humeral fracture nonunion. Methods A total of 71 patients with nonunion of humeral fractures were enrolled in this study from March 2014 to October 2015. All subjects were treated with compression plate internal fixation and autologous iliac bone grafting. The effect was investigated. Results All patients achieved bone healing, good alignment and contraposition following internal fixation. The average bone healing time was（11.5±2.4） months. There were no cases of intramedullary needle loosening, bending and rupture during the healing process. The function of the elbow joint revealed that 30 cases of excellent（42.3%）, 36 cases of good（50.7%）, 4 cases of better（5.6%）, and 1 case of poor（1.4%）, the excellent and good rate was 93.0%. The flexion, elbow flexion and MEPS score of the elbow were significantly better than those before treatment（P 〈 0.05）. The satisfaction showed that 41 cases of satisfactory（57.7%）, 24 cases of satisfied（33.8%）, 6 cases of unsatisfied, and the satisfaction rate was 91.5%. Conclusion Locking of pressurized steel plates with internal fixation and autogenous iliac bone grafting have a great effect in the treatment of humeral fracture nonunion, worthy of clinical application.
