摘 要:目的探讨研究骨髓涂片病态巨核细胞在小儿血液病中的临床价值。方法选择符合标准的小儿血液病的骨髓涂片105例,使用常规的瑞氏染色进行处理,并对每例骨髓涂片进行巨核细胞的计数,以及对淋巴样小巨细胞进行计数,以计算病态巨核细胞所占比例与淋巴样小巨细胞在巨核细胞中所占的比例。结果全片巨核细胞均数最高的为ITP组,显著性高于正常对照组及其他组别(P<0.05);HA、CML、MDS、ET、PV组中全片巨核细胞均数均显著性高于正常对照组(P<0.05);AA、ANLL、恶性肿瘤组中全片巨核细胞均数均显著性低于正常对照组(P<0.05);IDA组中全片巨核细胞均数与正常对照组相比没有显著性差异(P>0.05)。MDS组中全片淋巴样小巨核细胞均数最多,显著性高于其他组别(P<0.05);良性血液病组别与正常对照组中全片淋巴样小巨核细胞均数没有显著性差异(P>0.05)。结论通过观察血液病患儿骨髓涂片中巨核细胞的数量,对患儿血液病的诊断具有重要的意义,其可为良恶性血液病鉴别提供依据。Objective To integrative the clinical value of bone marrow smears of abnormal megakaryocyte in diagnosis of hematological diseases in children. Methods A total of 105 bone marrow smears of abnormal megakaryocyte coming from children patients with hematological diseases were chosen in the study. All the bone marrow smears were processed by using Wright stain To count the number of megakaryocytes in every bone marrow smear,and the number of small lymphoid cells in each case was also counted. Moreover the abnormal megakaryocyte proportion and lymphoid small megakaryocytes proportion were calculated. Results The maximum number of megakaryocyte in one smear was regarded as ITP group,which was significantly higher than that of control group and the other groups( P 〈0. 05). The number of megakaryocyte in HA,CML,MDS,ET,PV groups were significantly higher than that in control group( P〈 0. 05),however,the numbers of megakaryocyte in AA,ANLL and malignant tumor groups were significantly lower than those in control group( P 〈0. 05).There were no significant differences in the numbers of megakaryocyte between IDA group and control group( P〉 0. 05). The numbers of lymphoid micromegakaryocyte in MDS group were the most,which were significantly higher than those in the other groups( P〈 0. 05). There were no significant differences in the numbers of megakaryocyte between benign hematologic disease groups and control group( P〉 0. 05). Conclusion It is quite important in diagnosis of hematological diseases in children to observe the number of megakaryocytes in bone marrow smears,which can provide basis for differential diagnosis between benign and malignant hematological diseases.
