综合护理干预措施在老年肺癌放化疗患者中的应用效果分析

摘　　要：目的探讨综合护理干预措施对老年肺癌放化疗患者的应用价值。方法并接受放化疗的老年肺癌患者120例为研究对象,随机分为对照组和观察组,每组60例。对照组采用常规护理措施,观察组在常规护理基础上采用综合性护理干预,比较2组临床疗效、免疫功能、依从情况、并发症发生情况及护理满意度。结果护理后,观察组总有效率、完全依从率、护理满意度明显高于对照组,并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。2组护理前CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+、IgA、IgM、IgG、NK水平比较差异无统计学意义(P>0.05);2组护理后CD_3^+、CD_4^+、CD_4^+/CD_8^+、IgA、IgM、IgG、NK水平较对照组明显升高,CD_8^+水平较对照组明显降低,差异有统计学意义(P<0.05),且观察组护理后以上指标改善程度优于对照组(P<0.05)。结论老年肺癌放化疗患者采用综合护理干预措施不仅能够有效提高临床疗效、完全依从率、护理满意度,降低并发症发生率,还能改善患者免疫功能,值得临床推广。Objective To investigate the application effects of comprehensive nursing intervention in elderly patients with lung cancer after chemoradiotherapy. Methods A total of 120 elderly patients with lung cancer who underwent chemoradiotherapy in our hospital from June 2015 to Dec 2017 were randomly divided into observation group（ n = 60） and control group（ n = 60）. The patients in control group were treated by routine nursing,however,the patients in observation group,on the basis of control group,were treated by comprehensive nursing intervention. After treatment the curative effects,chemotherapy compliance,innunity function complications and nursing satisfaction degree were observed and compared between two groups. Results After nursing intervention,the total effective rate,complete compliance rate and nursing satisfaction degree in observation group were significantly higher than those in control group,however the incidence rate of complications in observation group was significantly lower than that in control group（ P 〈0. 05）. Before nursing intervention,there were no significant differences in CD＋3,CD＋4,CD＋8,CD＋4/CD＋8,IgA,IgM,IgG and NK between two groups（ P〉 0. 05）,however,after nursing intervention,the levels of CD3＋,CD4＋,CD4＋/CD8＋,IgA,IgM,IgG and NK in observation group were significantly higher than those in control group,but the levels of CD8＋ in observation group were significantly lower than those in control group,moreover,the improvement degree of these indexes in observation group was superior to that in control group（ P 〈0. 05）.Conclusion The application of comprehensive nursing intervention in elderly patients with lung cancer after chemoradiotherapy can not only increase the clinical curative effects,chemotherapy compliance and nursing satisfaction degree,decrease the incidence rate of complications,but also can improve the immune function of patients,therefor,which is worthy of clinical promotion.
