摘 要:目的探讨高分辨率CT(high-resolution computed tomography,HRCT)在鼻骨骨折中的应用价值。方法回顾性分析2015年1月—2016年4月确诊为鼻骨骨折的患者45例,其中男性30例,女性15例;年龄18~61岁,平均27.2岁。道路交通伤20例,钝器伤17例,摔伤8例。同时选取正常志愿者40例,男性23例,女性17例;年龄19~60岁,平均28.2岁。分别行鼻骨X线侧位和HRCT扫描,比较两种检查方法诊断价值。结果 45例患者中,X线检查确诊鼻骨骨折31例(68.89%),HRCT确诊鼻骨骨折43例(95.56%);40例正常者中,X线检查误诊为鼻骨骨折2例(5.00%),HRCT误诊为鼻骨骨折1例(2.50%)。HRCT的灵敏度为95.56%,明显高于X线检查的68.89%,差异有统计学意义(P<0.05);HRCT与X线检查特异度分别为95.00%和97.50%,差异无统计学意义(P>0.05)。结论在鼻骨骨折诊断中,HRCT具有较高的应用价值,有助于病例的诊断,减少漏诊。Objective To investigate the application value of high resolution CT( HRCT) in nasal bone fracture. Methods Retrospective analysis was conducted in 45 patients with fractures of the nasal bone diagnosed in our hospital from Jan. 2015 to Apr. 2016,including 30 males and 15 females,with an average age of 27. 2 years( mean,18 to 61 years). There were 20 road traffic injuries,17 blunt injuries,and 8 falling injuries. At the same time,40 normal volunteers were selected,including 23 males and 17 females,with an average of 28. 2 years( range,19 to 60 years). The X-ray lateral view of the nasal bone and the HRCT scan were performed respectively,and the diagnostic value of the two examination methods was compared. Results Among 45 patients,31 cases( 68. 89%)were diagnosed as nasal bone fractures by X-ray examination,and 43 cases( 95. 56%) were diagnosed as nasal bone fractures by HRCT. Among 40 normal subjects,2 cases( 5. 00%) were misdiagnosed as nasal bone fractures by Xray examination and 1 case( 2. 50%) by HRCT. The sensitivity of HRCT was 95. 56%,which was significantly higher than 68. 89% of X-ray examination,with statistical differences( P 0. 05). The specificity of HRCT and Xray examination was 95. 00% and 97. 50%,respectively,and the difference was not statistically significant( P 0. 05). Conclusion In the diagnosis of nasal bone fractures,HRCT has a high application value,which helps diagnosis and reduces missed diagnosis.
