摘 要:目的探讨振幅整合脑电图(a EEG)在重度高胆红素血症新生儿的远期体格和智力发育评估中的临床价值。方法选择2014年3月至2015年5月新乡市中心医院新生儿科收治的重度高胆红素血症患儿68例,根据a EEG结果将患儿分为振幅正常组(n=20)、振幅轻度异常组(n=25)和振幅重度异常组(n=23)。比较3组患儿6、12、24月龄时的体格发育、智能发育指数(MDI)、心理运动发育指数(PDI)及Gessell量表检测结果。结果振幅正常组和振幅轻度异常组患儿6、12、24月龄时体质量和身长高于振幅重度异常组(P<0.05);振幅正常组与振幅轻度异常组患儿6、12、24月龄时体质量、身长比较差异均无统计学意义(P>0.05)。3组患儿6月龄时头围比较差异无统计学意义(P>0.05);振幅正常组和振幅轻度异常组患儿12、24月龄时头围大于振幅重度异常组(P<0.05);振幅正常组和振幅轻度异常组患儿12、24月龄时头围比较差异无统计学意义(P>0.05)。3组患儿12、24月龄时MDI和PDI高于6月龄(P<0.05),24月龄时MDI和PDI高于12月龄(P<0.05)。振幅正常组和振幅轻度异常组患儿6、12、24月龄时MDI和PDI较振幅重度异常组显著增加(P<0.05);振幅正常组与振幅轻度异常组患儿6、12、24月龄时MDI和PDI比较差异无统计学意义(P>0.05)。振幅正常组和振幅轻度异常组患儿12、24月龄时适应性行为、大运动行为、精细动作行为、语言行为和个人-社交行为方面均显著优于振幅重度异常组(P<0.05);振幅正常组与振幅轻度异常组患儿12、24月龄时上述各指标比较差异均无统计学意义(P>0.05)。结论重度高胆红素血症新生儿a EEG振幅轻度异常者体格及智力发育可基本达到正常水平,振幅重度异常患儿远期体格及智力发育落后。Objective To investigate the value of amplitude integrated electroencephalogram( a EEG) in the assessment of long-term physical and mental development in neonates with severe hyperbilirubinemia. Methods Sixty-eight neonates with severe hyperbilirubinemia in the Central Hospital of Xinxiang from March 2014 to May 2015 were selected. All patients were divided into normal amplitude group( n = 20),amplitude mild abnormal group( n = 25) and amplitude severe abnormal group( n = 23) according to the results of a EEG. Physical development,mental development index( MDI),psychomotor development index( PDI) and Gessell scale test results were compared among the three groups at 6,12 and 24 months old. Results The weight and body length of children in normal amplitude group and amplitude mild abnormal group were significantly higher than those in the amplitude severe abnormal group at 6,12,24 months old( P 〈 0. 05); there was no statistic difference in the weight and body length of children between normal amplitude group and amplitude mild abnormal group at 6,12,24 months old( P 〉 0. 05). There was no statistic difference in the head circumference of children among the three groups at 6 months old( P 〉 0. 05); the head circumference of children in normal amplitude group and amplitude mild abnormal group was significantly higher than that in the amplitude severe abnormal group at 12,24 months old( P 〈 0. 05); there was no statistic difference in the head circumference of children between normal amplitude group and amplitude mild abnormal group at 12,24 months old( P 〉 0. 05). The MDI,PDI of children at 12,24 months old were significantly higher than those at 6 months old in the three groups( P 〈 0. 05); the MDI,PDI of children at 24 months old were significantly higher than those at 12 months old in the three groups( P 〈 0. 05).
