摘　　要：目的探讨手术切除联合放射治疗、术后局部注射A型肉毒毒素治疗瘢痕疙瘩的临床疗效及A型肉毒毒素的应用价值。方法将研究对象分为A(手术+放疗)、B(手术+放疗+术后即刻术口周围注释肉毒毒素)、C(手术+放疗+术后即刻及术后3-6月术口周围分别注射肉毒毒素)三组,术后第6个月及第12个月随访患者的疗效。结果治疗后6个月及12个月,A、B两组组的瘢痕评分差异无统计学意义,C组评分均低于A、B两组。治疗后12个月,A、B两组有效率无统计学差异,C组有效率高于A、B两组。结论手术联合术后放疗和局部重复注射A型肉毒毒素治疗瘢痕疙瘩有较好的临床疗效,明显减轻瘢痕疙瘩患者的痒痛等症状,并且安全性好,值得在临床上推广应用。Objective To investigate the clinical efficacy of botulinum toxin type A combined with operation and radiation therapy for the treatment of keloid and the application value of botulinum toxin. Methods The subjects were divided into A（surgery ＋ radiotherapy）, B（surgery ＋ radiotherapy ＋ immediately postoperative botulinum toxin injection）, C（surgery ＋ radiotherapy ＋ immediately postoperative and postoperative 3-6 months botulinum toxin injection around the incision）,the efficacy was followed up at 6 months and 12 months after operation. Results At 6 months and 12 months after treatment, there was no significant difference in the scores of the keloids between the A and B groups. The scores in the C group were lower than those in the A and B groups. After 12 months of treatment, there was no statistical difference between the two groups in the effective rates of A and B. The effective rate in group C was higher than that in group A and B. Conclusion Surgery combined with postoperative radiotherapy and repeatedly local injection of botulinum toxin type A in the treatment of keloids was effective and safe.. It can significantly reduce the symptoms such as itching and pain in patients with keloids, and it is safe and worthy of clinical application. It should be wildly used in clinics.
