摘　　要：目的探索一种用自体肋软骨来做鼻尖成形术的一种较好的方法。方法选择第7肋软骨,取出约3-4cm长的肋软骨,片状切开成长条片状,用一个较厚的长片状软骨做鼻尖的抬高,用两条片状软骨条做鼻尖的延长,同时雕刻肋软骨成帽状移植物和盾牌状移植物作为鼻尖的修饰。结果术后患者鼻尖外形较好,鼻尖表现点突出,无肋软骨移植物移位和变形的出现。取肋软骨术中未见气胸及血肿的并发症发生。术后胸壁未见明显手术瘢痕。结论应用肋软骨行鼻尖成形术,和耳软骨和鼻中隔软骨相比,可以提供足够的软骨量和强度,可以形成一个稳定的支架以保持鼻尖外形的稳定性。效果良好,是值得推广的一种方法。Objective To explore a preferable approach of rhinoplasty using autologous costal cartilage.Methods A 3—4 cm long costal cartilage was taken out mostly from th 7 th rib and then divided into several parts and shapes.A big cartilage bars for collumellar elevation and2 cartilage barsfor nasal tip elongation.A shield graft and a cap-shaped graft can be used on nasal alar cartilage dome for moulding. Results All the patients gain satisfactory results. Aesthetic parameter of nose tip can be obtained preferably.No such complications as implant dislocation、cartilage distortion、pneumothorax and hemothorax were observed.The scar was not obvius on walls of the chest. Conclusions Costal cartilage compared with auricular cartilage and nasal septal cartilage can provide sufficient materials to make up a stabile cartilage bracket.It is a preferable approach of rhinoplasty to popularize.
