肘前内侧入路张力带固定治疗尺骨冠状突骨折的疗效分析

摘　　要：目的评估肘前内侧入路张力带固定治疗尺骨冠状突骨折的临床疗效。方法选取2012年2月至2016年6月我科收治的尺骨冠状突骨折患者16例,其中男11例,女5例,年龄18~52岁,平均38岁。致伤原因:高处摔伤8例,交通事故伤8例。受伤部位:左侧6例,右侧10例,均为闭合性骨折。根据Regan-Morrey分型,I型5例,II型8例,III型3例。采用肘前内侧入路张力带固定进行治疗。结果术后切口均I期愈合,无血管、神经损伤及伤口感染发生,本组病例随访12~36个月,平均23个月,术后6个月肘关节屈伸活动度为(110±5)°,前臂旋转活动度为(120±15)°。肘关节HSS评分结果优10例,良5例,中1例,优良率达93.75%。结论尺骨冠状突骨折选择肘前内侧入路张力带固定可靠,手术创伤小,术后恢复好,住院花费少,可在临床推广。Objective To evaluate the effects of anteriomedial approach for the fixation of ulna coronoid process fracture with tension band. Methods Between February 2012 and December 2016, 16 patients（ 11 males and 5 females; aged 18-52 years, with an average of 38 years） with ulna coronoid process fracture were treated with operation by anteriomedial approach and fixed with tension band. Causes of injuries： 8 cases of high falling; 8 cases of traffic accident. Injury locations： 6 cases on the left side; 10 cases on the right; all were closed fractures. According to the Regan-Morrey classification： 5 cases of type I; 8 cases of type II; and 3 cases of type III. Results All incisions healed by the first intention. No blood vessel, nerve injury, or infections occurred. The patients were followed up for 12 to 36 months（ mean： 23 months）. The elbow joint flexion and extension ROM was（ 110 ±5） ° and the forearm rotation ROM 6 months after operation was（ 120 ± 15） °. Elbow HSS score was excellent in 10 cases, good in 5 cases, fair in 1 case, the excellent and good rate was 93.75%. Conclusions Anteriomedial approach for the fixation of ulna coronoid process fracture with tension band has the advantages of reliable clinical outcomes, less trauma, less cost, good recovery. It should be promoted in clinic.
