三四级手术量及其占比作为出国培训效果评价指标的研究

摘　　要：目的通过对北京市某三甲医院中长期出国开展临床技术培训人员出国前后手术量和三四级手术量占比变化的分析,为评价出国培训人员临床水平提供客观指标依据。方法随机选取某院4个外科系临床科室,每科随机选出3名有出国开展中长期临床技术培训经历的医师,及其相应科室同年资医师各7名,共计40名临床医师。调取在2012年1月1日-2017年12月31日期间40名医师管理手术患者的7374份出院病案首页。采用回顾性调查分析方法,对比分析出国培训人员出国前后1年内三四级手术量的变化以及手术量占科室同年资医师手术总量百分比。采用SPSS 17.0统计软件进行统计学分析,P<0.05为差异有统计学意义。结果 12名出国培训医师的1级和2级手术数量在出国培训前后均无显著性差异(P>0.05);3级和4级的手术数量显著上升(P<0.05)。在出国前后手术量占比的统计中,12名代表性医师在经历出国培训后,3级和4级的手术量占比有明显上升(P<0.05)。结论出国人员在国外经过先进的专业技能培训,回国后能够开展本专业高难度手术,从而提高学科人才培养和技术发展。可以将临床医师三四级手术量及其占比变化情况作为其出国学习效果的评价指标之一。Objective To providing objective indicators for assessing the clinical level of returnees through the analysis of the changes in the surgical level and the amount of surgery before and after a long-term overseas clinical training. Methods 3 representative doctors each from 4 different surgical departments in a grade III-A hospital in Beijing who had been go abroad for clinical training, and 7 doctors each from every corresponding departments, which added up to 40 doctors in total, were enrolled in this retrospective study. 7374 medical record home pages of the patients treated by the 40 doctors from January 1st, 2012 to December 31st, 2017 were obtained. Comparing and analyzing the changes of the operation level and the amount of surgeries within 1 year before and after leaving China, and using the SPSS 17.0 statistical software for statistical analysis. P〈0.05 was considered as a statistically significant difference. Results There was no significant difference in the number of level 1 and level 2 surgeries among the 12 representative doctors before and after training abroad （P^0.05）. After the overseas clinical training, the number of operations at level 3 and level 4 of 12 representative doctors increased significantly （P〈0.05）. In the statistics on the percentage of surgeries performed by the doctors among the whole department at the same year before and after going abroad, 12 representative doctors experienced a significant increase in the surgery ratio at level 3 and level 4 after training abroad （P〈0.05）. Conclusion The average amount and surgery ratio of level 3 and level 4 surgeries can be used as one of the evaluation indicators for evaluating the effectiveness of overseas study for doctors.
