分泌型卷曲相关蛋白5与慢性阻塞性肺疾病严重程度的关系

摘　　要：目的探讨分泌型卷曲相关蛋白5(sfrp5)在评估慢性阻塞性肺疾病(COPD)严重程度中的价值。方法选择2016年2月至2017年2月就诊于上海市第六人民医院东院呼吸内科门诊就诊的COPD稳定期患者370例作为COPD组,体检中心健康体检者205例作为对照组;根据COPD综合评估分组方法把COPD组分4组:A组98例,B组71例,C组75例,D组126例,行肺功能检查;利用酶联免疫法(ELISA)测定血清sfrp5水平。结果 COPD患者sfrp5水平显著低于正常对照组,各组COPD患者sfrp5水平存在显著差异:A组>B组>C组>D组。相关性分析显示:COPD患者血清sfrp5与BMI、甘油三酯、FBG均呈显著负相关(r值分别为-0.346、-0.244、-0.501,均P<0.01),与炎症因子IL-6、TNF-α水平也呈显著负相关(r值分别为-0.401、-0.382,均P<0.01),与肺功能参数FEV1/FVC%、FEV1占预计值%均呈显著正相关(r值分别为0.472、0.579,均P<0.01)。结论血清sfrp5与包括气流受限程度、急性发作风险、症状严重程度在内的COPD综合评估的病情严重程度密切相关,且与炎症相关,表明sfrp5反映不同疾病程度并可能参与COPD发病机制。Objective To investigate the value of secreted frizzled-related protein-5（ sfrp5） in evaluating the severity of chronic obstructive pulmonary disease（ COPD）. Methods From February 2016 to February 2017,370 patients with stable COPD selected from the Outpatient Department of our hospital were taken as the experimental groups,and 205 healthy persons selected from the Physical Examination Center of our hospital were taken as the control group. Based on the comprehensive evaluation of COPD,the patients were divided into 4 experimental groups： group A（ n = 98）,group B（ n = 71）,group C（ n = 75） and group D（ n =126）. Serum sfrp5 levels were measured by enzyme-linked immunosorbent assay. Results The levels of sfrp5 in the patients with COPD were significantly lower than that of the control group. There were significant differences in sfrp5 levels in the COPD patients： group A〉 group B〉 group C 〉group D. Correlation analysis showed that the serum sfrp5 of the COPD patients was significantly negatively correlated with the body mass index（ BMI）,triglyceride and fasting blood glucose（ FBG,r =-0.346,-0.244 and-0.501,respectively,P〈0.01）. There was also a significant negative correlation between sfrp5 and pro-inflammatory factors interleukin-6（ IL-6） and tumor necrosis factor（ TNF）-α（ r =-0.401 and-0.382,respectively,P〈0.01）. And sfrp5 was positively correlated with the lung function parameters,such as the ratio of the forced expiratory volume in 1 second（ FEV1） to the forced vital capacity（ FVC%） and FEV1%（ r = 0. 472 and 0. 579,respectively,P〈0.01）. Conclusion Serum Sfrp5 is related well with the airflow limitation,acute exacerbation risk,and symptom severity in the COPD patients,which suggests that sfrp5 may reflect the severity of disease and may be involved in the pathogenesis of COPD.
