晕听区针刺结合手法复位治疗水平半规管良性阵发性位置性眩晕的临床观察

摘　　要：目的:通过对单纯手法复位与晕听区针刺结合手法复位对向地性眼震型水平半规管良性阵发性位置性眩晕的治疗效果比较,探索晕听区针刺结合手法复位的治疗效果。方法:所有病例来自本附属医院,共60例,按照随机数表法将其分为观察组和对照组,每组各30例。观察组采用针刺晕听区结合手法复位治疗,主穴取晕听区、百会穴配合对症选穴;手法复位采用Gufoni法(向健侧)治疗。结果:观察组和对照组组间治疗前DHI评分比较,无统计学意义(P>0.05);治疗后1天、7天,两组DHI评分与治疗前比较有统计学意义(P<0.05);治疗后1天、7天两组间情绪评分、功能评分及DHI总分比较,有统计学意义;治疗后1天两组间躯体评分比较无统计学意义(P>0.05);治疗后7天两组间躯体评分比较有显著性统计学意义(P<0.01);观察组治疗总有效率为93.3%,对照组为86.7%,两组比较无统计学意义(P>0.05)。结论:晕听区针刺配合手法复位治疗向地性眼震型水平半规管良性阵发性位置性眩晕具有较好临床疗效。Objective： To explore the therapeutic effect of needling vertigo-auditory zone combined with manual reposition in treatment of benign geotropism horizontal semicircular canal vertigo（ HSCV）. Methods： 60 patients were randomly divided into the observation group（ n = 30） and the control group（ n = 30）. The control group was treated with manual reposition（ Gufoni method,2-3 times）; on which basis,the observation group was also treated with needling vertigo-auditory zone,Baihui（ GV20） and other points based on syndrome differentiation. Results： In terms of DHI score,there was no significant difference between the two groups before treatment（ P〈0. 05）; there was statistical difference after 1 d and 7 d treatment in the two groups（ P〈0. 05）.There were statistical differences in POMS,functional scale and DHI between the two groups after 1 d and 7 d treatment（ P〈0. 05）. In terms of FDIP,no difference showed up in the two groups after 1 d treatment（ P〈0. 05）; however,the difference showed up after 7 d treatment（ P〈0. 01）. In terms of the total effective rate,it was 93. 3% in the observation group,and it was 86. 7% in the control group,but there was no significant differences between the two groups（ P〉0. 05）. Conclusion： Needling vertigo-auditory zone combined with manual reposition has better curative effect on benign HSCV.
