摘  要:目的探讨深低温及体外循环创伤对失血性休克猪血脂水平的影响。方法对健康成年巴马香猪18头在全身麻醉呼吸机辅助条件下行颈总动脉和颈内静脉插管建立体外循环,转流降温至直肠温度15℃,暂停体外循环90 min后恢复体外循环并逐渐复温,待直肠温度恢复到术前水平后脱离体外循环,直至脱离呼吸机,观察巴马香猪血脂相关指标甘油三酯(TG)、血总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及高密度脂蛋白胆固醇(HDL-C)等的变化情况。结果实验猪术中低温休克期至术后第7天的血脂相关指标血清TC、TG、LDL-C水平均低于术前2 h,低温休克期至术后第3天HDL-C水平均低于术前2 h,差异均有统计学意义(P<0.05)。血脂各指标水平在术中低温休克期最低。结论深低温及体外循环创伤可致失血性休克猪的血脂水平明显下降。Objective To explore the effect of deep hypothermic extracorporeal circulation on blood lipid of hemorrhagic shock swine.Methods Totally 18 swines under general anesthetic were established extracorporeal circulation with common carotid artery and internal jugular vein intubation assisted by the ventilator,the extracorporeal circulation was suspended for 90 minutes until the rectal temperature dropped to 15℃,then the extracorporeal circulation was recovered and the temperature was risen. When the rectal temperature was as the same as the the preoperative level,the extracorporeal circulation and the ventilator was over,the level of triglyceride( TG),total cholesterol( TC),low density lipoprotein cholesterin( LDL-C) and high density lipoprotein cholesterol( HDL-C) were observed.Results The levels of TC,TG,HDL-C and LDL-C in the process of experiment were lower than those at 2 hours before experiment,the level HDL-C from hypothermic shock stage to 3 days was lower than that at 2 hours before experiment,there were significant difference( P〈0. 05). At the intraoperative hypothermic shock stage,TC、TG、LDL-C and HDL-C were the lowest. Conclusion Deep hypothermia and extracorporeal circulation trauma can lead to a significant decrease of TC,TG,HDL-C and LDL-C of shock swine.
