宫腔粘连患者子宫内膜组织跨膜前列腺雄激素诱导蛋白1和转化生长因子β1的表达情况

摘　　要：目的探讨跨膜前列腺雄激素诱导蛋白1(prostate transmembrane protein androgen induced 1,PMEPA 1)和转化生长因子β1(transforming growth factor-β1,TGF-β1)在宫腔粘连(intrauterine adhesions,IUA)患者子宫内膜组织中的表达量及相关性。方法选取武汉大学人民医院妇产科2016年6月至2017年12月因IUA行宫腔镜手术治疗的患者50例为粘连组,20例因不孕症行宫腔镜手术的非IUA患者为对照组,采集两组患者子宫内膜组织,根据粘连的评分标准评分并分级,粘连组又分为轻度组(10例)、中度组(19例)、重度组(21例),收集的内膜组织采用qRT-PCR和Western blot检测PMEPA 1和TGF-β1的mRNA和蛋白的表达量并在蛋白表达水平作两者相关性分析。结果 (1)PMEPA 1 mRNA的表达:中、重度粘连组低于对照组和轻度粘连组,重度组低于中度组,差异均有统计学意义(t=7.719、12.84、2.614,P<0.0001、<0.0001、<0.05);轻度组与对照组比较差异无统计学意义(P>0.05)。(2)PMEPA 1蛋白的表达:轻、中、重度粘连组均低于对照组(t=2.886、6.669、15.47,P<0.05、<0.001、<0.0001),重度组低于中度组(t=5.119,P<0.0001)。(3)TGF-β1的mRNA和蛋白的表达:粘连组高于对照组(t=4.988、8.816、9.0514,P均<0.001)。(4)PMEPA1和TGF-β1在中、重度组中呈负相关关系(r=-0.6342、-0.8407,P=0.0035、<0.0001)。结论 PMEPA 1和TGF-β1可能参与IUA的发生发展。Objective To detect the expression of PMEPA1 and TGF-β1 in endometrium of patients with intrauterine adhesions（ IUA） and analyze the correlation between the two indexes.Methods 50 IUA patients and 20 non-IUA patients（ control group） were selected from Renmin Hospital of Wuhan University from June 2016 to December 2017 to acquire the endometrium.According to the points-scoring system of AFS,the IUA patients were divided into 3 groups,the light group（ 10 patients）,the moderate group（ 19 patients） and the severe group（ 21 patients）.Rt-RCR and Western blot were used to detecte the mRNA and protein of PMEPA1 and TGF-β1 and analyze the correlation between the two indexes at protein level.Results（1） The expression of PMEPA 1 mRNA was lower in the moderate and severe adhesion group than in the control group and the mild adhesion group,and the severe group was lower than the moderate group（ t = 7.719,12.84,2.614,P 0.0001,P 0.0001, 0.05）.（2） PMEPA 1 protein expression： the light,moderate,and severe adhesion groups were lower than the control group（ t = 2.886,6.669,15.47,P 0.05,0.001, 0.0001）,and the severe group was lower than the middle group（ t = 5.119,P 0.0001）.（3） mRNA and protein expression of TGF-β1： The adhesion group was higher than the control group（ t = 4.988,8.816,9.0514,P all 0.001）.（4） PMEPA1 and TGF-β1 were negatively correlated in the moderate and severe groups（ r =-0.6342,-0.8407,P = 0.0031, 0.0001）.Conclusion PMEPA1 and TGF-β1 may participate in the occurrence and development of IUA.
