超声诊断急诊腹部闭合性损伤内脏破裂出血的价值

摘　　要：目的探讨急诊腹部闭合性损伤内脏破裂出血应用超声诊断的临床价值。方法以2015年1月—2018年3月我院急诊科49例腹部闭合性损伤内脏破裂患者为研究对象,全部患者均行急诊超声检查,回顾性分析患者超声影像资料,观察声像图特点,统计超声诊断内脏破裂出血准确率。结果 49例患者经临床综合检查或手术病理,确诊脾破裂37例,肝破裂4例,肾挫伤6例,复合脏器伤2例。以此为金标准,48例患者首次急诊超声检查即刻确诊,1例迟发性出血,再次超声复查明确诊断,诊断准确率100%。结论超声诊断腹部闭合性损伤内脏破裂出血准确率高,可作为急诊检查首选辅助方法在临床使用。Objective To investigate the clinical value of ultrasound diagnosis for visceral rupture bleeding in emergency closed abdominal trauma. Methods From January 2015 to March 2018, 49 patients with visceral rupture due to closed abdominal trauma in our emergency department were studied. All patients underwent emergency ultrasonography. The ultrasonographic data of the patients were retrospectively analyzed. The ultrasonographic features were observed and the accuracy of ultrasonography in diagnosis of visceral rupture and bleeding was statistically analyzed. Results 49 cases were diagnosed as splenic rupture in 37 cases, liver rupture in 4 cases, renal contusion in 6 cases and complex organ injury in 2 cases by comprehensive clinical examination or surgical pathology. 48 cases were diagnosed immediately after the first emergency ultrasound examination, 1 case of delayed hemorrhage, the diagnosis was confirmed again by ultrasound, the diagnostic accuracy was 100%. Conclusion Ultrasound diagnosis of abdominal closed injury with high accuracy of visceral rupture bleeding, can be used as an auxiliary method of emergency examination in clinical use.
