雌孕激素序贯治疗早孕药物流产子宫出血的临床分析

摘　　要：目的:探讨早孕药物流产子宫出血患者接受雌孕激素序贯治疗的临床效果。方法:选取2015年8月—2018年3月在我院接受药物流产的早孕患者102例,根据随机数字表法分为观察组51例和对照组51例。对照组采用常规止血、消炎治疗,观察组在对照组基础上加用雌孕激素序贯治疗,对比两组出血时间、激素水平及子宫内膜修复和清宫发生情况。结果:观察组药物流产后出血时间明显短于对照组,差异有统计学意义(P<0.05);观察组药流后7d时孕酮(Progesterone,P)水平低于对照组,雌二醇(Estradiol,E2)水平较高,差异有统计学意义(P<0.05);观察组子宫内膜修复率明显高于对照组,清宫率低于对照组,差异有统计学意义(P<0.05)。结论:雌孕激素序贯治疗可有效缩短早孕药物流产患者子宫出血时间,改善激素水平,降低清宫发生率。Objective： To investigate the clinical effect of sequential treatment of estrogen and progesterone in patients with uterine hemorrhage after early pregnancy. Methods： 102 cases of early pregnancy who received drug abortion from August 2015 to March 2018 were divided into observation group （n=51） and control group （n=51）. The control group was treated with routine hemostasis and anti-infammatory therapy, the observation group was treated with estradiol and progesterone on the basis of the control group. The bleeding time, hormone level, endometrial repair and uterine clearing were compared between the two groups. Results： The bleeding time after drug abortion in the observation group was signifcantly shorter than that in the control group （P〈0.05）. The levels of progesterone and estradiol E2 in the observation group were lower than those in the control group （P〈0.05）; the endometrial repair rate in the observation group was signifcantly higher than that in the control group （P〈0.05）; and the uterine clearing rate in the observation group was lower than that in the control group （P〈0.05）. Conclusion： Sequential treatment of estrogen and progesterone can effectively shorten the time of uterine bleeding, improve the hormone level and reduce the incidence of uterine clearing.
