摘  要:目的探讨控制性超促排卵治疗周期中卵泡发育速度的变化及其对妊娠结局的影响。方法选取计划生育宣传技术站接受IVF-ET的采用控制性超促排卵治疗的270例不孕症患者共270个周期,根据卵泡发育速度(v,mm/d)将患者分为A组(v<1 mm/d)、B组(1 mm/d≤v≤1.59 mm/d)、C组(v>1.59 mm/d),比较组间妊娠结局的差异。结果 270个周期中卵泡发育速度(1.29±0.65)mm/d,B、C组卵母细胞成熟率高于A组,A、C组优质胚胎率高于B组,但组间比较差异无统计学意义(P>0.05),B组妊娠率明显高于A、C组,差异有统计学意义(P<0.05)。B组平均获卵数、胚胎移植数、胚胎种植率、出生率明显高于A、C组,差异有统计学意义(P<0.05)。B组卵裂率、早期流产率低于A、C组,差异无统计学意义(P>0.05)。结论控制性促超排卵治疗周期中卵泡发育速度应控制于最佳范围内(1~1.59 mm/d),因其与妊娠结局具有密切的关系,卵泡发育速度是衡量胚胎质量的主要指标。Objective To investigate the changes of follicular development velocity during controlling super ovarian stimulation cycle and its effects on pregnancy outcome. Methods A total of 270 female patients with infertility(270 cycles)who received in vitro fertilization-embryo transfer(IVF-ET) by means of controlling super ovarian stimulation treatment in our hospital from July 2014 to July 2015 were enrolled in the study. According to follicular development velocity(v,mm/d),these patients were divided into group A(v 1 mm/d),group B(1 mm/d≤v≤1. 59 mm/d) and group C(v 1. 59 mm/d),the differences of pregnancy outcomes were compared among the three groups. Results Among 270 cycles with follicular development velocity being(1. 29 ± 0. 65) mm/d,the oocyte maturation rate in group B and group C was higher than that in group A,moreover,the high quality embryo rate in group A and group C was higher than that in group B,however,there was no significant difference between two groups each other(P〈0. 05). The pregnancy rate in group B group was significantly higher than that in group A and group C(P〈0. 05). Moreover the average ovum number,embryo transfer number,embryo planting rate,birth rate in group B were significantly higher than those in group A and group C(P〈0. 05). In addition the cleavage rate,early abortion rate in group B were lower than those in group A and group C,however,there was no significant difference among groups(P〈0. 05). Conclusion The follicular development velocity during controlling super ovarian stimulation treatment should be controlled in optimal range of 1 ~ 1. 59 mm/d,because which is closely correlated with pregnancy outcomes,and follicular development velocity is the main index to evaluate embryo quality.
