摘 要:目的探讨孟鲁司特联合阿奇霉素对小儿上呼吸道肺炎支原体感染患儿免疫功能指标的影响。方法选取108例支原体感染诱发上呼吸道肺炎患儿,随机分为对照组和研究组各54例。2组患儿均接受常规的基础治疗,在此基础上,对照组患儿接受阿奇霉素治疗,研究组接受孟鲁司特联合阿奇霉素治疗。比较2组患儿临床症状与体征的缓解时间、住院时间及治疗前后免疫功能变化。结果研究组患儿咳嗽消失时间、退热时间、喘憋消失时间、啰音消失时间、住院时间均显著短于对照组(P<0.05);治疗后,2组患儿的免疫球蛋白指标IgG及T细胞亚群指标CD4^+、CD4^+/CD8^+水平均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);治疗后,2组患儿的免疫球蛋白指标IgM、IgA及T细胞亚群指标CD8^+水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05)。结论孟鲁司特联合阿奇霉素治疗小儿上呼吸道肺炎支原体感染效果显著,能有效缓解症状及体征,改善机体免疫功能。Objective To study effect of montelukast combined with azithromycin on immune function indicators in children with upper respiratory tract mycoplasma pneumonia. Methods A total of 108 children with upper respiratory tract pneumonia caused by mycoplasma infection in our hospital were selected as research subjects. All the patients were divided into control group and study group according to random number table method,with 54 cases in each group. Both groups received routine basic treatment. At the same time,the control group received azithromycin for treatment,and the study group was given montelukast combined with azithromycin for treatment. The remission time of clinical symptoms and signs,the duration of hospitalization and the changes of immune function of the two groups were compared. Results The cough disappearing time,antipyretic time,asthma disappearing time,rales disappearing time and hospitalization time in the study group were all lower than that in the control group( P〈0. 05). The levels of T lymphocyte subsets such as CD4^+and CD4^+/CD8^+,immunoglobulin( IgG) in both groups after treatment were higher than treatment before,and the study group was higher than the control group( P〈0. 05). The IgM,IgA and CD8^+in both groups were lower than treatment before,and the study group was lower than the control group( P〈0. 05). Conclusion Montelukast combined azithromycin treatment for children with upper respiratory tract mycoplasma pneumonia has significant efficacy,and it can effectively relieve symptoms and signs,improve immune function.
