摘 要:目的观察前列地尔脂微球载体注射液(凯时)配伍甲钴胺及α-硫辛酸对糖尿病周围神经病变(DPN)患者神经传导速率与氧化应激的影响。方法选取2015年1月至2016年12月收治的DPN患者116例,随机分为对照组(59例)和观察组(57例)。对照组患者口服甲钴胺片与α-硫辛酸胶囊,观察组患者在对照组患者治疗基础上加用前列地尔脂微球载体注射液,疗程为8周。结果两组基线特征比较,差异无统计学意义(P>0.05);观察组治疗后感觉传导速率(SNCV)和运动传导速率(MNCV)提高值显著高于对照组[腓总神经:SNCV,(5.91±1.55)m/s比(4.85±1.19)m/s,MNCV,(6.57±1.63)m/s比(5.61±1.47)m/s;正中神经:SNCV,(6.36±1.76)m/s比(5.22±1.52)m/s,MNCV,(7.07±2.05)m/s比(6.17±1.85)m/s](P<0.05);观察组治疗后血清丙二醛(MDA)下降值[(1.72±0.70)μmol/L比(1.25±0.57)μmol/L]、超氧化物歧化酶(SOD)升高值[16.42±4.84)U/mL比(14.37±4.25)U/mL]、谷胱甘肽过氧化物酶(GSH-Px)升高值[(8.24±2.51)U/mL比(6.46±2.18)U/mL],均显著高于对照组(P<0.05);观察组治疗后多伦多神经病变评分系统(TCSS)评分下降值为(2.95±0.82)分,显著高于对照组的(2.42±0.65)分(P<0.05)。结论在甲钴胺及α-硫辛酸治疗DPN基础上加用前列地尔脂微球载体注射液,能协同提高神经传导速率与机体抗氧化水平,增进临床疗效。Objective To observe the effects of Lipo-PGE1 Injection combined with mecobalamin and α-lipoic acid on neuro-electrophysiology and oxidative stress in patients with diabetic peripheral neuropathy( DPN).Methods Totally 116 DPN patients admitted to our hospital from January 2015 to December 2016 were selected and randomly divided into the control group( 59 cases) and the observation group( 57 cases).The control group was treated with Mecobalamin Tablets and α-Lipoic Acid Capsules,on this basis,the observation group was treated with Lipo-PGE1 Injection.The treatment course was 8 weeks.Results There were no significant differences in baseline characteristics between the two groups( P〈0.05).After treatment,the increased values of sensory conduction velocity( SNCV),motor conduction velocity( MNCV) in the observation group were significantly higher than the control group [ Nervus peroneus communis:SNCV( 5.91 ± 1.55) m/s vs.( 4.85 ± 1.19) m/s,MNCV( 6.57 ± 1.63) m/s vs.( 5.61 ± 1.47) m/s; Median nerve:SNCV( 6.36 ± 1.76) m/s vs.( 5.22 ± 1.52) m/s,MNCV( 7.07 ± 2.05) m/s vs.( 6.17 ± 1.85) m/s]( P〈0.05).After treatment,the decreased value of MDA[( 1.72 ± 0.70) μmol/L vs.( 1.25 ± 0.57) μmol/L ],increased value of SOD [( 16.42 ± 4.84) U/mL vs.( 14.37 ± 4.25) U/mL ],increased value of GSH-Px [( 8.24 ± 2.51) U/mL vs.( 6.46 ± 2.18) U/mL ] in the observation group were higher than those in the control group( P〈0.05).After treatment,the decreased score of Toronto clinical scoring system( TCSS) in the observation group was( 2.95 ± 0.82) points,which was significantly higher than( 2.42 ± 0.65) points in the control group( P〈0.05).Conclusion On the basis of mecobalamin and α-lipoic acid in the treatment of DPN,adding Lipo-PGE1 Injection can synergistically improve the nerve conduction velocity and the antioxidation level,and it also can improve the clinical efficacy.
