摘　　要：目的探讨注射用胸腺肽α_1治疗小儿呼吸机相关性肺炎(ventilator-associated pneumonia,VAP)的疗效及对患儿免疫功能的影响。方法选取医院儿科重症加强护理病房2015年5月至2017年9月收治的VAP患儿130例,按治疗方法的不同分为对照组和研究组,各65例。对照组患儿予以常规治疗,研究组患儿在对照组治疗基础上加用胸腺肽α_1治疗,疗程为2周。结果研究组总有效率为84.62%,明显高于对照组的66.15%(P<0.05);治疗后,研究组患儿机械通气时间及住院时间均较对照组患儿明显缩短(P<0.05);两组患儿治疗后CD_3^+细胞分数、CD_4^+细胞分数、CD_4^+/CD_8^+比值及自然杀伤(NK)细胞分数均明显高于治疗前(P<0.05),且研究组患儿均明显高于对照组患儿(P<0.05);两组患儿治疗过程中均未发生明显不良反应。结论注射用胸腺肽α_1治疗小儿VAP临床疗效显著,可缩短患儿机械通气时间及住院时间,改善患儿的免疫功能,且安全可靠,值得临床推广。Objective To investigate the clincial effect of Thymosin α1 for Injection in the treatment of children patients with ventilator-associated pneumonia（ VAP） and its effect on immune function.Methods Totally 130 children patients with VAP admitted to the pediatric Intensive Care Unit in our hospital from May 2015 to September 2017 were selected and divided into the control group and the study group according to different treatment methods,65 cases in each group.The control group was given routine treatment,on this basis,the study group was treated with Thymosin α1 for Injection.The course was 2 weeks.Results The total effective rate of the study group was 84.62%,which was significantly higher than 66.15% of the control group（ P〈0.05）.After treatment,the mechanical ventilation time and hospital stay in the study group were significantly shorter than those in the control group（ P〈0.05）.After treatment,the cell fraction of CD3＋,CD4＋,CD4＋/CD8＋ and NK in the two groups were significantly higher than those before treatment,and those in the study group were significantly higher than those in the control group（ P〈0.05）.No obvious adverse reactions occurred in the two groups during the treatment.Conclusion Thymosin α1 for Injection is effective and safe in the treatment of children patients with VAP,it can shorten the mechanical ventilation time and hospital stay,improve the immune function,which is worthy of clinical promotion.
