氯吡格雷联合高压氧对进展型脑梗死患者神经功能、日常生活能力和血流动力学的影响

摘　　要：目的探讨氯吡格雷联合高压氧对进展型脑梗死患者神经功能、日常生活能力和血流动力学的影响。方法选取医院2015年2月至2017年9月收治的进展型脑梗死患者124例,按治疗方法的不同分为对照组和观察组,各62例。两组患者均予以常规对症支持治疗,对照组患者给予口服氯吡格雷治疗,观察组患者在对照组患者治疗基础上联合高压氧治疗,疗程为30 d。结果观察组总有效率为96.77%,明显高于对照组的82.26%(P<0.05);治疗后,两组患者美国国立卫生研究院卒中量表(NIHSS)评分明显降低,改良Barthel指数(MBI)评分明显升高(P<0.05),且观察组变化更明显(P<0.05);观察组患者治疗10 d及30 d后收缩期峰值流速(PSV)、舒张末期血流速率(EDV)、血流速率(V_m)、血管搏动指数(PI)均较治疗前及对照组升高(P<0.05),阻力指数(RI)较治疗前及对照组降低(P<0.05);对照组治疗30 d后PSV,EDV,V_m,PI均较治疗前升高,而RI较治疗前降低(P<0.05)。结论氯吡格雷联合高压氧治疗进展型脑梗死临床疗效显著,可有效促进患者神经功能和日常生活能力的恢复,明显改善患者的血流动力学水平。Objective To investigate the effect of clopidogrel combined with hyperbaric oxygen on nerve function,activity of daily living and hemodynamics in patients with progressive cerebral infarction.Methods Totally 124 patients with progressive cerebral infarction admitted to our hospital from February 2015 to September 2017 were selected and divided into the control group and the observation group according to different treatment methods,62 cases in each group.The two groups were given conventional symptomatic and supportive treatment,on this basis,the control group was given clopidogrel,and the observation group was treated with hyperbaric oxygen on the basis of the control group.The course of treatment was 30 d.Results The total effective rate of the observation group was 96.77%,which was significantly higher than 82.26% of the control group（ P〈0.05）.After treatment,the NIHSS scores were significantly decreased and the MBI scores were significantly increased in the two groups（ P〈0.05）,and the change in the observation group was more significant（ P〈0.05）.After 10,30 d of treatment,the PSV,EVD,Vmand PI were significantly increased and the RI was significantly decreased than that before treatment and that in the observation group（ P〈0.05）.After 30 d of treatment,the PSV,EDV,Vm,PI were significantly increased,but RI was significantly decreased in the control group than those before treatment（ P〈0.05）.Conclusion Clopidogrel combined with hyperbaric oxygen is effective in the treatment of patients with progressive cerebral infarction,it can effectively promote the recovery of neurological function and ability of daily living,and significantly improve the hemodynamics level of patients.
