规范化耳后扩张器置入术在分期耳再造术中的应用分析

摘　　要：目的:探讨耳后扩张器置入术的规范化手术操作,从而减少术后并发症的发生。方法:通过对2016年1月-2017年7月笔者科室收治的132例先天性小耳畸形行耳后扩张器置入术患者的手术操作流程及术后并发症发生率进行总结分析,明确规范化手术流程操作对减少并发症发生的必要性。结果:扩张器置入术后共出现0例血肿,6例扩张器感染,6例扩张器外露,0例面神经损伤。131例患者顺利完成二期耳再造术,1例患者因扩张器外露行扩张器取出。结论:耳后皮肤扩张能够获得充分的全层皮肤为二期置入软骨外耳支架做准备,掌握精准的规范化手术操作及注水流程、客观并全面地进行术前谈话及宣教,能够有效预防并发症的发生,降低不良影响。Objective To investigate the standardization of surgery procedures about tissue expander implantation at retroauricular, which could reduce the postoperative complications. Methods The operation flow and the incidence of postoperative complications were summarized and analyzed in 132 cases of congenital microtia with postauricular dilator in our hospital from January 2016 to July 2017, confired the necessity to standardize the surgery procedures. Results There were 0 cases of hematoma, 6 cases of dilator infection, 6 cases of dilator exposure, and 0 cases of facial nerve injury after dilator placement. There were 131 patients underwent two stage auricular reconstruction, and one patient had expander removal because of exposure. Conclusion Skin and soft tissue expanding could obtain adequate skin tissue to envelops the cartilage frame of two stage surgery. Accurate and standardized procedures of surgery and expander injection, had objective and comprehensive communication and education with patient and their family could effectively prevent the occurrence of complications and reduce the adverse effects of complications.
